Abstract

GAMBARAN ASUHAN KEPERAWATAN IBU PRIMIGRAVIDA TRIMESTER III DENGAN KESIAPAN PENINGKATAN PENGETAHUAN TENTANG PEMBERIAN ASI EKSKLUSIF TAHUN 2019 ABSTRAK Cakupan pemberian ASI eksklusif terendah adalah di Puskesmas I Denpasar Barat. Persiapan laktasi dilakukan pada masa kehamilan dan bertujuan untuk membantu keberhasilan proses pemberian ASI eksklusif. Penelitian ini bertujuan untuk mengetahui gambaran asuhan keperawatan ibu primigravida trimester III dengan kesiapan peningkatan pengetahuan tentang pemberian ASI eksklusif di Puskesmas I Denpasar Barat tahun 2019. Jenis penelitian yang digunakan adalah deskriptif dengan rancangan studi kasus. Tehnik yang digunakan dalam pengumpulan data penelitian adalah observasi dan dokumentasi yang dilakukan pada 2 rekam medis pasien dengan kesiapan peningkatan pengetahuan tentang pemberian ASI eksklusif. Hasil penelitian pada pengkajian terdapat kesamaan dengan teori, diagnosa keperawatan yang ditegakkan adalah diagnosa kebidanan, perencanaan tidak didokumentasikan di dalam rekam medis pasien, implementasi dilakukan namun tidak didokumentasikan karena terbatasnya format asuhan pada rekam medis, serta evaluasi menggunakan SOAP dengan perbedaan pada planning. Penelitian ini menunjukkan bahwa terdapat perbedaan data dengan teori yang dijadikan acuan oleh peneliti. Hal ini menunjukkan bahwa pendokumentasian asuhan keperawatan dipengaruhi oleh teori yang ditetapkan sebagai standard oleh petugas kesehatan di Puskesmas I Denpasar Barat dan peneliti. Diharapkan dalam mendokumentasikan asuhan sebaiknya mempergunakan format asuhan yang meliputi pengkajian, diagnosa, perencanaan, implemtasi, dan evaluasi tanpa menghilangkan salah satu komponen dan mendokumentasikan seluruh tindakan yang telah dilakukan. Kata kunci : Asuhan Keperawatan, Kesiapan Peningkatan Pengetahuan, Ibu Primigravida Trimester III, ASI eksklusif DESCRIPTION OF NURSING PRIMIGRAVIDA TRIMESTER III WITH READINESS OF KNOWLEDGE ABOUT EXCLUSIVE BREASTFEEDING 2019 ABSTRACT The lowest coverage of exclusive breastfeeding is in the puskesmas 1 West Denpasar. Lactation preparation is carried out during pregnancy and aims to help the success of the exclusive breastfeeding process. This study aims to describe the primigravida trimester III nursing care with readiness to increase knowledge about exclusive breastfeeding in puskesmas 1 West Denpasar 2019. The type of research used was descriptive with a case study design. The technique used in collecting research data was observation and documentation conducted on 2 medical records of patients with readiness to increase knowledge about exclusive breastfeeding. The results of the study on the study have similarities with the theory, the nursing diagnoses that are enforced are midwifery diagnoses, planning is not documented in the patient's medical record, implementation is carried out but not documented because of the limited format of care in medical records, and evaluation using SOAP with different planning. This study shows that there are several differences in data with the theory used as a reference by researchers. This shows that the documentation of nursing care is influenced by the theory set as standard by the health workers at the West Denpasar Health Center I and researchers.it is expected that documenting care should use the form of guidance which includes the assessement, nursing diagnoses,planning, implementation and evaluation without eliminating misunderstandings compenent and document all action taken.
