Abstract

Nursing Care for Child Patient M with Ineffective Airway Clearance Due to Pulmonary Tuberculosis in the Sakura Ward at Buleleng Regional General Hospital 2026. ABSTRACT Pulmonary tuberculosis is an infectious disease that remains a health problem, especially in children, and often causes ineffective airway clearance due to increased secretion production. Data from the Sakura Ward at Buleleng Regional General Hospital showed 15 cases of pulmonary tuberculosis in children in 2023 and 2024, and 8 cases in 2025. This case report aimed to describe nursing care for a child with ineffective airway clearance caused by pulmonary tuberculosis in the Sakura Ward of Buleleng Regional General Hospital in 2026. The method used was a descriptive with a case study approach through the application of the nursing process, including assessment, nursing diagnosis, planning, implementation, and evaluation. The assessment results of An. M, aged 8 years and 10 months, showed shortness of breath, productive cough, increased sputum production, additional breath sounds, irregular breathing patterns, and a respiratory rate of 30 breaths/minute. The nursing diagnosis established was ineffective airway clearance related to airway hypersecretion. The main interventions provided included airway management, semi-Fowler position, effective coughing exercises, and collaboration in pharmacological therapy. Evaluation showed improvement in the patient’s condition. It was concluded that airway management interventions were effective in overcoming ineffective airway clearance problems. Health workers are advised to optimize standard-based interventions and improve family education and involvement in the care of patients with pulmonary tuberculosis. ASUHAN KEPERAWATAN PADA AN.M DENGAN BERSIHAN JALAN NAPAS TIDAK EFEKTIF AKIBAT TUBERKULOSIS PARU DI RUANG SAKURA RUMAH SAKIT UMUM DAERAH BULELENG TAHUN 2026 ABSTRAK Tuberkulosis paru merupakan penyakit infeksi yang masih menjadi masalah kesehatan, terutama pada anak, dan sering menimbulkan gangguan bersihan jalan napas akibat peningkatan produksi sekret. Data di Ruang Sakura RSUD Buleleng menunjukkan kasus tuberkulosis paru pada anak sebanyak 15 kasus pada tahun 2023 dan 2024, serta 8 kasus pada tahun 2025. Laporan kasus ini bertujuan menggambarkan asuhan keperawatan pada anak dengan masalah bersihan jalan napas tidak efektif akibat tuberkulosis paru di Ruang Sakura RSUD Buleleng tahun 2026. Metode yang digunakan adalah deskriptif dengan pendekatan studi kasus melalui penerapan proses keperawatan yang meliputi pengkajian, diagnosis keperawatan, perencanaan, implementasi, dan evaluasi. Hasil pengkajian pada An.M usia 8 tahun 10 bulan menunjukkan sesak napas, batuk berdahak, peningkatan sputum, bunyi napas tambahan, pola napas tidak teratur, dan frekuensi napas 30 x/menit. Diagnosis keperawatan yang ditegakkan yaitu bersihan jalan napas tidak efektif berhubungan dengan hipersekresi jalan napas. Intervensi utama yang diberikan meliputi manajemen jalan napas, posisi semi fowler, latihan batuk efektif, dan kolaborasi terapi farmakologis. Evaluasi menunjukkan kondisi pasien membaik. Disimpulkan bahwa intervensi manajemen jalan napas efektif mengatasi masalah bersihan jalan napas tidak efektif. Disarankan tenaga kesehatan mengoptimalkan intervensi berbasis standar serta meningkatkan edukasi dan keterlibatan keluarga dalam perawatan pasien tuberkulosis paru. Kata kunci: tuberkulosis paru, bersihan jalan napas tidak efektif, asuhan keperawatan.
