Abstract
DESCRIPTION OF NURSING CARE MOTHER ON STAGE III SPONTANEOUS PARTUS WITH RISK POSTPARTUM HEMORRHAGE IN DELIVERY ROOM WANGAYA HOSPITAL ABSTRACT Obstetric hemorrhage remains a major cause of maternal death in the United States, and 54% to 93% of these deaths has a great opportunity to be prevented. Active management of the third stage of labor has been shown to reduce the risk of postpartum haemorrhage (PPH) is more than 1000 cc. This study aims to know the description of nursing care at the third stage normal delivery mothers with postpartum hemorrhage risk in Wangaya Hospital. This type of research is descriptive and the techniques of data collection is documentation guidelines. Number of subjects are 2 document. The results showed there is a difference between the format of nursing and midwifery because researchers used the theory of reference that refers to assessment of bleeding risk while nursing diagnosis in patients using the format document assessment refers to the midwifery. Diagnosis is formulated based on the diagnosis of obstetrik followed by data subjective and objective while the nursing formulated the diagnosis based on the guidelines Indonesia Standards Nursing Diagnoses. Interventions are formulated based on objectives and outcomes do active management of the third stage, vaginal bleeding checked. Implementation is done in according with the intervention is do active management of the third stage. Evaluation was found in two patients using the SOAP document is complete impression of spontaneous delivery of the placenta and vaginal bleeding 100 cc. Keywords: Nursing care, Spontaneous partus, Third stage of labour, Risk of bleeding GAMBARAN ASUHAN KEPERAWATAN PADA IBU PARTUS SPONTAN KALA III DENGAN RISIKO PERDARAHAN POST PARTUM DI RUANG BERSALIN RSUD WANGAYA ABSTRAK Manajemen aktif kala III persalinan telah terbukti mengurangi risiko perdarahan pascapersalinan (PPH). Penelitian ini bertujuan mengetahui gambaran asuhan keperawatan pada ibu partus spontan kala III dengan risiko perdarahan post partum di Ruang Bersalin RSUD Wangaya. Jenis penelitian yang digunakan adalah deskriptif dengan pendekatan studi kasus. Teknik pengumpulan data yang digunakan yaitu studi dokumentasi. Jumlah subyek yaitu dua ibu bersalin kala III. Hasil penelitian menunjukkan dalam pengkajian pada dokumen pasien sesuai dengan teori pengkajian persalinan kala III. Diagnosa dirumuskan berdasarkan diagnosa kebidanan berdasarkan kerangka pikir Varney diikuti oleh data subyektif dan data obyektif sedangkan profesi keperawatan merumuskan diagnosa berdasarkan pedoman Standar Diagnosis Keperawatan Indonesia (SDKI). Intervensi dirumuskan berdasarkan tujuan dan kriteria hasil yaitu lakukan manajemen aktif kala III. Implementasi dilakukan sesuai dengan intervensi yaitu melakukan manajemen aktif kala III. Evaluasi didapatkan pada dokumen kedua pasien menggunakan SOAP yaitu plasenta lahir spontan kesan lengkap dan perdarahan pervaginam tidak aktif.
