ASUHAN KEPERAWATAN BERSIHAN JALAN NAPAS TIDAK EFEKTIF DENGAN KOMBINASI PEMBERIAN POSISI ORTHOPNEA DAN HAND HELD FAN THERAPY PADA PASIEN PPOK DI RUANG JEPUN RSUD BALI MANDARA TAHUN 2026 Aura Anastasia Angelica Goguyu Program Studi Profesi Ners Poltekkes Kemenkes Denpasar Email: [email protected] ABSTRAK PPOK merupakan gangguan pernapasan kronis yang ditandai dengan hambatan aliran udara yang bersifat progresif dan tidak sepenuhnya reversibel. Kondisi ini sering menimbulkan masalah keperawatan berupa bersihan jalan napas tidak efektif akibat sesak nafas, penurunan ventilasi paru, bunyi napas tambahan, infeksi saluran pernapasan, retensi sekret. Salah satu intervensi nonfarmakologis yang dapat digunakan adalah kombinasi pemberian posisi orthopnea dan hand held fan therapy. Penelitian ini bertujuan untuk menggambarkan asuhan keperawatan bersihan jalan napas tidak efektif dengan penerapan kombinasi posisi orthopnea dan hand held fan therapy pada pasien PPOK di ruang jepun RSUD Bali Mandara. Metode yang digunakan adalah metode deskriptif dengan pendekatan studi kasus pada satu pasien, melalui tahapan pengkajian, diagnosis, perencanaan, implementasi, dan evaluasi. Hasil menunjukkan bahwa setelah dilakukan intervensi selama 3x24 jam, terjadi perbaikan pola napas yang ditandai dengan penurunan frekuensi napas, penurunan pola napas, penurunan rasa dispnea, penurunan sputum yang berlebih, penurunan bunyi suara napas tambahan, serta peningkatan kenyamanan pasien. Pemberian posisi orthopnea membantu memaksimalkan ekspansi paru dan menurunkan beban kerja pernapasan, sedangkan hand held fan therapy memberikan sensasi aliran udara yang membantu mengurangi persepsi sesak napas. Simpulan penelitian ini adalah kombinasi posisi orthopnea dan hand held fan therapy efektif sebagai intervensi nonfarmakologis dalam meningkatkan bersihan jalan napas pada pasien PPOK serta membantu meningkatkan kenyamanan dan kemampuan adaptasi pasien terhadap sesak napas. Keywords: PPOK, Posisi Orthopnea, Hand Held Fan Therapy, Bersihan Jalan Napas Tidak Efektif NURSING CARE FOR INEFFECTIVE BREATHING PATTERNS WITH A COMBINATION OF ORTHOPNEIC POSITIONING AND HAND-HELD FAN THERAPY IN COPD PATIENTS IN THE JEPUN ROOM OF BALI MANDARA REGIONAL HOSPITAL IN 2026 ABSTRACT COPD is a chronic respiratory disorder characterized by progressive and non- reversible airflow obstruction. This condition often causes nursing problems such as ineffective airway clearance due to shortness of breath, decreased pulmonary ventilation, adventitious breath sounds, respiratory tract infections, and secretion retention. One non-pharmacological intervention that can be used is a combination of orthopnea positioning and hand-held fan therapy. This study aims to describe nursing care for ineffective airway clearance by applying a combination of positioning and hand-held fan therapy to COPD patients in the Japanese ward of Bali Mandara Regional Hospital. The method used is a descriptive method with a case study approach on one patient, through the stages of assessment, diagnosis, planning, implementation, and evaluation. The results showed that after 3x24 hours of intervention, there was an improvement in breathing patterns characterized by a decrease in respiratory frequency, a decrease in breathing patterns, a decrease in dyspnea, a decrease in excessive sputum, a decrease in adventitious breath sounds, and an increase in patient comfort. Orthopnea positioning helps maximize lung expansion and reduces the workload of breathing, while hand-held fan therapy provides a sensation of airflow that helps reduce the perception of shortness of breath. The conclusion of this study is that the combination of orthopnea position and hand-held fan therapy is effective as a non- pharmacological intervention in improving airway clearance in COPD patients and helping to improve patient comfort and adaptation to shortness of breath.
