Abstract

MIDWIFERY CARE FOR A 30-YEAR-OLD MULTIGRAVID MOTHER "WR"FROM 20 WEEKS OF GESTATION TO 42 DAYS OF POSTPARTUM ABSTRACT Pregnancy, childbirth, and the postpartum period carry a risk of complications that can seriously impact the health of the mother and baby. The role of midwives is crucial in identifying and preventing these complications through continuous care. This report aims to determine the results of midwifery care implementation for a 30-year-old multigravida mother, "WR," and her baby, who received comprehensive and continuous midwifery care from 20 weeks of gestation to 42 days postpartum. The case determination method was through interviews, examinations, and documentation. During pregnancy, the mother was cared for according to the 12T standards, with complementary care in the form of warm ginger drinks, pregnancy exercises, pregnancy yoga, and perineal massage before and during labor. The labor process proceeded normally and no complications were found. Complementary care provided included relaxation, breathing regulation, and lower back massage, so that the mother's labor proceeded smoothly and without perineal rupture. The baby was born spontaneously, a girl, cried immediately, and weighed 3005 grams. The mother's postpartum period proceeded physiologically, with complementary care in the form of oxytocin massage, postpartum exercises, and Kegel exercises. The mother is confident about using the IUD. The baby received standard neonatal care, SHK screening, and complementary care, including infant massage. The care provided to mother "WR" was comprehensive and continuous, according to standards. Peran bidan sangat penting untuk mengidentifikasi dan mencegah komplikasi tersebut melalui asuhan yang berkesinambungan. Laporan ini bertujuan mengetahui hasil penerapan asuhan kebidanan pada ibu “WR” umur 30 tahun multigravida beserta bayi yang menerima asuhan kebidanan secara komprehensif dan berkesinambungan dari umur kehamilan 20 minggu sampai dengan 42 hari masa nifas. Metode penentuan kasus melalui wawancara, pemeriksaan, serta dokumentasi. Selama kehamilan ibu diasuh sesuai standar 12T, asuhan komplementer berupa minuman jahe hangat, senam hamil, yoga hamil dan pijat perineum menjelang dan selama persalinan. Proses persalinan berlangsung secara normal dan tidak ditemukan penyulit. Komplementer yang diberikan relaksasi pengaturan pernafasan dan pijat punggung bawah sehingga persalinan ibu berjalan dengan lancar dan tanpa ruptur perineum. Bayi lahir spontan, perempuan, segera menangis dan berat badan lahir 3005 gr. Masa nifas ibu berlangsung secara fisiologis, asuhan komplementer berupa pijat oksitosin, senam nifas dan senam kegel. Ibu mantap untuk menggunakan KB IUD. Bayi mendapatkan asuhan standar pelayanan neonatus, skrining SHK, serta asuhan komplementer pijat bayi. Asuhan yang diberikan pada ibu “WR” sudah diberikan sesuai dengan standar secara komprehensif dan berkesinambungan. Kata kunci : asuhan berkesinambungan, kehamilan, persalinan, nifas, neonatus
