Abstract

NURSING CARE FOR INEFFECTIVE AIRWAY CLEANSING WITH ACTIVE CYCLE OF BREATHING TECHNIQUE INTERVENSION WITH COPD PATIENT IN GENERAL BANGLI HOSPITAL ABSTRACT COPD is a chronic respiratory symptom that causes progressive and persistent airflow obstruction. One of the nursing problems experienced is ineffective airway clearance which can be overcome with active cycle of breathing technique. The purpose of this study was to determine nursing care for ineffective airway clearance in COPD patients with active cycle of breathing technique. The descriptive research design was a case study with a nursing process approach that reported one case of nursing care carried out in the Cempaka Room, Bangli Hospital on April 10-13, 2025. The results of the assessment showed signs/symptoms of ineffective cough, excessive sputum, wheezing breath sounds and dry rhonchi, dyspnea, restlessness, breathing frequency and breathing pattern changes, hyperaerated lung x-ray results, history of COPD since 3 years ago. The nursing diagnosis of ineffective airway clearance related to retained secretions is evidenced by ineffective cough, excessive sputum, wheezing breath sounds and dry rhonchi, dyspnea, restlessness, breathing frequency and breathing pattern changes. Nursing plan with increased airway clearance outcomes and airway management interventions, effective coughing exercises, respiratory monitoring, and active cycle of breathing technique were given for 3 days, 2 times a day for 10-30 minutes. Nursing evaluation showed that the objectives were achieved with increased effective coughing, decreased sputum production, wheezing, dyspnea, and restlessness, improved respiratory rate and breathing patterns. Active cycle of breathing technique therapy can help improve airway clearance in COPD patients and can be used as an independent nursing action based on complementary. Keywords: Active Cycle of Breathing Technique, COPD, Ineffective Airway Clearance ASUHAN KEPERAWATAN BERSIHAN JALAN NAPAS TIDAK EFEKTIF PADA PASIEN PPOK DENGAN ACTIVE CYCLE OF BREATHING TECHNIQUE DI RSUD BANGLI ABSTRAK PPOK merupakan gejala penapasan kronik yang menyebabkan penyumbatan aliran udara yang progresif dan persisten. Salah satu masalah keperawatan yang dialami adalah bersihan jalan napas tidak efektif yang dapat diatasi dengan active cycle of breathing technique. Tujuan penelitian untuk mengetahui asuhan keperawatan bersihan jalan napas tidak efektif pada pasien PPOK dengan active cycle of breathing technique. Desain penelitian deskriptif jenis studi kasus dengan pendekatan proses keperawatan yang melaporkan satu kasus asuhan keperawatan yang dilaksanakan di Ruang Cempaka RSUD Bangli tanggal 10-13 April 2025. Hasil pengkajian didapatkan tanda/gejala batuk tidak efektif, sputum berlebih, bunyi napas wheezing dan ronkhi kering, dispnea, gelisah, frekuensi napas dan pola napas berubah, hasil rontgen hiperaerasi paru, riwayat PPOK sejak 3 tahun yang lalu. Diagnosis keperawatan bersihan jalan napas tidak efektif berhubungan dengan sekresi yang tertahan dibuktikan dengan batuk tidak efektif, sputum berlebih, bunyi napas wheezing dan ronkhi kering, dispnea, gelisah, frekuensi napas dan pola napas berubah. Rencana keperawatan dengan luaran bersihan jalan napas meningkat dan intervensi manajemen jalan napas, latihan batuk efektif pemantauan respirasi, serta active cycle of breathing technique diberikan selama 3 hari sebanyak 2 kali sehari selama 10-30 menit. Evaluasi keperawatan menunjukkan tujuan tercapai dengan hasil batuk efektif meningkat, produksi sputum, wheezing, dispnea, dan gelisah menurun, frekuensi napas dan pola napas membaik. Terapi active cycle of breathing technique dapat membantu meningkatkan bersihan jalan napas pada pasien PPOK serta dapat digunakan sebagai tindakan mandiri perawat berbasis komplementer.
