Abstract

NURSING CARE OF INEFFECTIVE AIRWAYS CLEARANCE WITH BALLOON BLOWING EXERCISE INTERVENTION IN COPD PATIENTS AT JEPUN ROOM BALI MANDARA HOSPITAL ABSTRACT COPD is the third leading cause of death in the world, and caused 3.23 million deaths in 2019. Based on a preliminary study conducted at Bali Mandara Hospital and based on medical record data at Bali Mandara Hospital, there has been an increase in cases of hospitalized patients with COPD in the last 2 years, namely in 2023 as many as 63 patients increasing to 80 patients in 2024. Early diagnosis and treatment tailored to needs are important in reducing the risk of death. This paper aims to determine nursing care for COPD patients who experience ineffective airway clearance in the Jepun Room of Bali Mandara Hospital using a descriptive research type. The sample is COPD patient. The types of data collected in this study are primary data and secondary data, the method of data collection in this study uses interviews, observations, physical examinations and documentation studies. The results of the study found patient data saying shortness of breath, ineffective cough, difficulty expelling phlegm, and there were additional breath sounds of rhonchi and wheezing, oxygen saturation 93%. The patient had been suffering from COPD for approximately seven years and had often relapsed in the last three months. The diagnosis formulated is ineffective airway clearance related to retained secretions. The planned intervention is airway management by focusing on providing modified breathing exercise therapy using the Balloon Blowing Exercise technique for three days. The results of the nursing evaluation carried out have been achieved where the patient's shortness of breath began to decrease, effective coughing increased, respiration became normal, wheezing and rhonchi decreased and oxygen saturation increased. Based on the results of this study, it is recommended that modified breathing exercise therapy using the Balloon Blowing Exercise technique be an alternative choice for nurses in providing interventions for COPD patients who experience ineffective airway clearance. Keywords: COPD, Ineffective Airway Clearance, Balloon Blowing Exercise ASUHAN KEPERAWATAN BERSIHAN JALAN NAPAS TIDAK EFEKTIF DENGAN INTERVENSI BALLOON BLOWING EXERCISE PADA PASIEN PPOK DI RUANG JEPUN RSUD BALI MANDARA ABSTRAK PPOK merupakan penyebab kematian nomor tiga di dunia, dan menyebabkan 3,23 juta kematian pada tahun 2019. Berdasarkan studi pendahuluan yang dilakukan di RSUD Bali Mandara dan berdasarkan data rekam medik di RSUD Bali Mandara terdapat peningkatan kasus pasien rawat inap dengan PPOK dalam 2 tahun terakhir, yaitu tahun 2023 sebanyak 63 pasien meningkat menjadi 80 pasien pada tahun 2024. Diagnosis dini dan perawatan yang disesuaikan dengan kebutuhan penting dalam mengurangi risiko kematian. Karya tulis ini bertujuan untuk mengetahui asuhan keperawatan pada pasien PPOK yang mengalami bersihan jalan napas tidak efektif di Ruang Jepun RSUD Bali Mandara menggunakan jenis penelitian deskriptif. Sampel adalah Pasien PPOK. Jenis data yang dikumpulkan pada penelitian ini merupakan data primer dan data sekunder, cara pengumpulan data dalam penelitian ini menggunakan wawancara, observasi, pemeriksaan fisik dan studi dokumentasi. Hasil penelitian pada pengkajian ditemukan data pasien mengatakan sesak, batuk tidak efektif, sulit mengeluarkan dahak, dan terdapat suara nafas tambahan ronki dan mengi, saturasi oksigen 93%. Pasien sudah menderita PPOK kurang lebih selama tujuh tahun dan sering kambuh dalam tiga bulan terakhir. Diagnosa yang dirumuskan yaitu bersihan jalan napas tidak efektif berhubungan dengan sekresi yang tertahan. Intervensi yang direncanakan adalah manajemen jalan napas dengan memfokuskan pada tindakan pemberian terapi modifikasi latihan pernafasan teknik Balloon Blowing Exercise selama tiga hari. Hasil evaluasi keperawatan yang dilakukan sudah tercapai dimana sesak pasien mulai berkurang, batuk efektif meningkat, respirasi menjadi normal, suara nafas mengi dan ronki menurun dan saturasi oksigen meningkat. Berdasarkan hasil penelitian ini disarankan agar terapi modifikasi latihan pernafasan teknik Balloon Blowing Exercise dijadikan sebuah alternatif pilihan bagi perawat dalam memberikan intervensi pada pasien PPOK yang mengalami bersihan jalan napas tidak efektif Kata Kunci : PPOK, Bersihan Jalan Napas Tidak Efektif, Balloon Blowing Exercise
