Abstract

NURSING CARE FOR INEFFECTIVE AIRWAY CLEARANCE WITH A COMBINATION OF DIAPHRAGMATIC BREATHING EXERCISE AND PURSED LIP BREATHING IN CHRONIC OBSTRUCTIVE PULMONARY DISEASE (COPD) PATIENTS AT KLUNGKUNG DISTRICT HOSPITAL IN 2025 ABSTRACT Chronic Obstructive Pulmonary Disease (COPD) is a significant public health problem in Indonesia. Retained secretions result in ineffective airway clearance. The purpose of this scientific paper is to determine the description of nursing care for ineffective airway clearance with a combination of diaphragmatic breathing exercise and pursed lip breathing in COPD patients. The study used a descriptive case study approach design which was carried out on May 7-9, 2025. The results of the study obtained data on ineffective cough, unable to cough, excessive sputum, shortness of breath, restlessness, wheezing, dry rhonchi, dyspnea, altered breathing frequency, and altered breathing pattern so that the patient was declared to have ineffective airway clearance problems. The problem was overcome with effective cough exercises, airway management, and combination therapy of diaphragmatic breathing exercise and pursed lip breathing. After intervention for 3 x 24 hours, airway clearance increased with the criteria for effective cough results increased, sputum production decreased, wheezing decreased, rhonchi decreased, dyspnea decreased, restlessness improved, breathing frequency improved, and breathing pattern improved. The conclusion of this study is that the provision of combination therapy of diaphragmatic breathing exercise and pursed lip breathing is effective in increasing airway clearance in patients with COPD and can be used as an alternative solution to the problem of ineffective airway clearance by nurses as an independent nursing action. ASUHAN KEPERAWATAN BERSIHAN JALAN NAPAS TIDAK EFEKTIF DENGAN KOMBINASI DIAPHRAGMATIC BREATHING EXERCISE DAN PURSED LIP BREATHING PADA PASIEN PENYAKIT PARU OBSTRUKTIF KRONIK (PPOK) DI RSUD KABUPATEN KLUNGKUNG TAHUN 2025 ABSTRAK Penyakit Paru Obstruktif Kronik (PPOK) merupakan masalah kesehatan masyarakat yang signifikan di Indonesia. Sekresi yang tertahan mengakibatkan bersihan jalan napas tidak efektif. Tujuan karya ilmiah ini adalah mengetahui gambaran asuhan keperawatan bersihan jalan napas tidak efektif dengan kombinasi diaphragmatic breathing exercise dan pursed lip breathing pada pasien PPOK. Penelitian menggunakan rancangan deskriptif pendekatan studi kasus yang dilaksanakan pada tanggal 7 sampai 9 Mei 2025. Hasil pengkajian didapatkan data batuk tidak efektif, tidak mampu batuk, sputum berlebih, sesak, gelisah, wheezing, ronkhi kering, dispnea , frekuensi napas berubah, dan pola napas berubah sehingga pasien dinyatakan mengalami masalah besihan jalan napas tidak efekif. Masalah diatasi dengan latihan batuk efektif, manajemen jalan napas,dan terapi kombinasi diaphragmatic breathing exercise dan pursed lip breathing. Setelah dilakukan intervensi selama 3 x 24 jam, bersihan jalan napas meningkat dengan kriteria hasil batuk efektif meningkat, produksi sputum menurun, wheezing menurun, ronchi menurun,dispnea menurun,gelisah membaik frekuensi napas membaik, dan pola napas mebaik. Simpulan dari penelitian ini, pemberian terapi kombinasi diaphragmatic breathing exercise dan pursed lip breathing efektif meningkatkan bersihan jalan napas pada pasien dengan PPOK dan dapat digunakan sebagai alternatif pemecahan masalah bersihan jalan napas tidak efektif oleh perawat sebagai suatu tindakan mandiri perawat. Kata Kunci : Bersihan Jalan Napas Tidak efektif , PPOK, Diaphragmatic Breathing Exercise, Pursed Lip Breathing.
