Abstract

ABSTRACT Maternal and infant mortality cases are still in the limelight in Indonesia. Mothers and children are family members who need to be prioritized in health efforts, in this case midwives have the authority to provide health services to mothers, children, and women’s reproductive health and family planning. This final project report aims to determine the application of obstetric care which is carried out comprehensively and continuously in “DU” mothers aged 22 years primigravida from 30 weeks 1 day to 42 days of puerperium. The data collection methods used are interviews, examinations, observations and documentation. Care is given from March to May 2022 which takes place physiologically. The upbringing of childbirth was carried out from the time I of delivery, but there was no progress in opening at the time I of the active phase and also experienced a weakened fetal heart rate and was diagnosed with fetal distress, so sc action was carried out. Changes in the lokhea, a decrease in the height of the uterine fundus, maternal lactation within normal limits, the sutures of the SC wound are already dry and cut on the 8 day post-SC, and the mother goes through a phase of psychological adaptation normally. Babies are born weighing 3420 grams, body length 50 cm with an apgar score of 7-9. The baby’s weight gain during 42 days of upbringing is 870 grams. The problems that occur during obstetric care of pregnancy, puerperium and baby last physiologically, however, the period of childbirth is pathological. Key words: Midwifery Upbringing; pregnancy; labor sc; baby ABSTRAK Kasus kematian ibu dan bayi masih menjadi pusat perhatian di Indonesia. Ibu dan anak merupakan anggota keluarga yang perlu mendapat prioritas dalam upaya kesehatan, dalam hal ini bidan memiliki kewenangan untuk memberikan pelayanan kesehatan pada ibu, anak, dan kesehatan reproduksi perempuan dan keluarga berencana. Laporan tugas akhir ini bertujuan untuk mengetahui penerapan asuhan kebidanan yang dilakukan secara komprehensif dan berkesinambungan pada ibu “DU” umur 22 tahun primigravida dari umur kehamilan 30 minggu 1 hari sampai 42 hari masa nifas. Metode pengumpulan data yang digunakan adalah wawancara, pemeriksaan, observasi serta dokumentasi. Asuhan diberikan dari bulan Maret sampai bulan Mei 2022 yang berlangsung secara fisiologis. Asuhan persalinan dilakukan dari kala I persalinan, namun tidak terjadi kemajuan pembukaan pada kala I fase aktif dan juga mengalami denyut jantung janin melemah dan di ahagia fetal distress, sehingga dilakukan tindakan SC. Perubahan lokhea, penurunan tinggi fundus uteri, laktasi ibu dalam batas normal, jahitan luka SC sudah kering dan di potong pada hari ke-8 pasca SC, dan ibu melewati fase adaptasi psikologis secara normal. Bayi lahir dengan berat 3420 gram, ahagia badan 50 cm dengan apgar score 7-9. Kenaikan berat badan bayi selama 42 hari asuhan yaitu 870 gram. Masalah yang terjadi selama asuhan kebidanan kehamilan, nifas dan bayi berlangsung fisiologis, namun masa persalinan berlangsung patologis.
