Abstract
Undernutrition is a condition in which the body’s nutritional needs are not met within a certain time, it caused our body system split food reserves that are under fat layer and organ layers. The main nursing problem of undernutrition child under five is nutritional deficiency, characterized by a weight loss at least 10% under the ideal range. This study aims to determine the description of nursing care in undernutrition children with nutritional deficiency at Public Health Centre of Tegallalang I on 12-20 April 2018. This is a descriptive type of research with case study design on two undernutrition child under five. The results showed that on nursing assement. The results showed on nursing assement were done anthropometric measurements include BB, TB and LK, health history assessment, immunization status and eating habits, no nursing diagnosis format and no nutritional deficiency diagnosis, no nursing planning format, some action on implementation were not implemented and evaluation is done every month, the criteria of the results evaluated the same as the theory and the status is still undernutrition. Nursing care documentation is influenced by the orientation of the service, the place of health service and the reference used as a reference.
