Abstract

Indonesia is undergoing an epidemiological transition in which changes in disease patterns are characterized by an increase in non-communicable diseases. Non-communicable diseases, especially hypertension. In the Gianyar Health Profile in 2018 the pattern of 10 diseases showed the second most cases in patients was hypertension with the number reaching 6,489 (13.97%). Although hypertension medicines are widely available, there are still many hypertension sufferers who do not carry out a healthy lifestyle so that blood pressure remains in a normal state. This is due to lack of knowledge, low education of hypertension sufferers to improve their lifestyle. The purpose of this research is to describe the nursing care of hypertensive patients by providing germas health education in fulfilling the readiness to increase knowledge in the work area of the Public Health Unit Sukawati I Gianyar. This type of research is a type of descriptive research with a case study design of two clients. Data collection methods with literature review and documentation conducted in Banjar Puseh and Tebuana, Sukawati District. The results showed both clients found signs and symptoms formulated nursing diagnosis that is the readiness to increase knowledge. The nursing plan is given health education with the Germas program. Implementation of nursing in 3 visits and 30 minutes each visit to provide health education germas. The results of the evaluation found that blood pressure did not increase, following the advice given, and the client was able to implement a healthy lifestyle with germas. Suggestions are expected clients to continue to implement a healthy lifestyle germas and seek information about healthy lifestyles in hypertension patients. Keywords: Knowledge Improvement, Healthy lifestyle Indonesia mengalami transisi epidemiologi yakni perubahan pola penyakit ditandai dengan terjadinya peningkatan adanya penyakit tidak menular (PTM). Penyakit tidak menular terutama hipertensi. Dalam Profil Kesehatan Gianyar Tahun 2018 pola 10 penyakit menunjukan kasus terbanyak kedua pada pasien adalah penyakit hipertensi dengan jumlah mencapai 6.489 (13,97%). Walaupun obat-obatan hipertensi sudah banyak tersedia, namun masih banyakpenderita hipertensi yang tidak menjalankan pola hidup sehat agar tekanan darah tetap dalam keadaan normal. Hal ini disebabkan karena kurangnya pengetahuan, rendahnya pendidikan penderita hipertensi untuk memperbaiki pola hidupnya. Tujuan dilakukannya penelitian ini adalah menggambarkan asuhan keperawatan pasien hipertensi dengan pemberian edukasi kesehatan germas dalam pemenuhan kesiapan peningkatan pengetahuan di wilayah kerja UPT Kesmas Sukawati I Gianyar. Jenis penelitian yang digunakan adalah jenis penelitian deskriptif dengan rancangan studi kasus terhadap dua klien. Metode pengumpulan data dengan literature review dan dokumentasi yang dilakukan di Banjar Puseh dan Tebuana, Kecamatan Sukawati. Hasil penelitian menunjukan kedua klien ditemukan tanda gejala dan dirumuskan diagsnosis keperawatan yaitu kesiapan peningkatan pengetahuan. Rencana keperawatan yaitu diberikan edukasi kesehatan dengan program germas. Implementasi keperawatan dalam 3 kali kunjungan dan 30 menit tiap kunjungannya untuk pemberian edukasi kesehatan germas. Hasil evaluasi ditemukan tekanan darah tidak meningkat, mengikuti anjuran yang diberikan, dan klien mampu menerapkan pola hidup sehat dengan germas. Saran diharapkan klien tetap menerapkan pola hidup sehat germas dan mencari informasi mengenai pola hidup sehat pada pasien hipertensi. Kata kunci: Peningkatan Pengetahuan, Pola hidup sehat
