Abstract

NURSING DESCRIPTION CARE IN POST PARTUM MOTHER WITH RISK OF INFECTION OF PERINIUM WOUND ABSTRACT In general, every mother wants her childbirth process to run smoothly and naturally, but often during labor the perinium trauma takes place. The increasing number of normal deliveries during the last three years at Wangaya Hospital in Denpasar City, resulted in an increased risk of infection in primigravida postpartum mothers caused by injury to the birth canal. This study determine the description of nursing care in post partum mothers with the risk of perinium wound infection. This type of research is descriptive with a case study design. The study was conducted using observation and documentation study techniques in two normal post partum patients in the Wangaya Hospital Dara Room. The results of this study include the studies obtained in the field, there are no gaps with the theory used by researchers as a reference for the two patients. Nursing diagnoses were not appointed in the Dara Room of Wangaya Hospital, but used midwifery diagnoses. In nursing planning there is a gap where in the patient documentation sheet there is no nursing plan but planning for post partum midwifery care. The implementation carried out is in accordance with the planned plan but has not documented in detail. Evaluation has been documented using the integrated patient development record consisting of SOAP but there are few gaps with the theory that the researcher uses as a reference, namely in the assessment section. Keywords: Nursing care, risk of infection, post partum mother   GAMBARAN ASUHAN KEPERAWATAN PADA IBU POST PARTUM DENGAN RESIKO INFEKSI LUKA PERINIUM ABSTRAK Pada umumnya setiap calon Ibu ingin proses persalinannya berjalan dengan lancar dan tanpa hambatan, tetapi seringkali selama proses persalinan berlangsung terjadi trauma perinium. Meningkatnya jumlah persalinan normal selama tiga tahun terakhir di RSUD Wangaya Kota Denpasar, mengakibatkan terjadinya peningkatan resiko infeksi pada ibu post partum primigravida yang disebabkan oleh perlukaan jalan lahir. Penelitian ini bertujuan untuk mengetahui gambaran asuhan keperawatan pada ibu post partum dengan resiko infeksi luka perinium. Jenis penelitin ini adalah deskriptif dengan rancangan studi kasus. Penelitian dilakukan dengan menggunakan teknik observasi dan studi dokumentasi pada dua orang pasien post partum normal di Ruang Dara RSUD Wangaya. Hasil penelitian ini antara lain pengkajian yang didapat di lapangan tidak terdapat kesenjangan dengan teori yang digunakan peneliti sebagai acuan pada kedua pasien. Diagnosa keperawatan tidak diangkat di Ruang Dara RSUD Wangaya, namun menggunakan diagnosa kebidanan. Pada perencanaan keperawatan terdapat kesenjangan dimana pada lembar dokumentasi pasien tidak terdapat rencana keperawatan melainkan perencanaan asuhan kebidanan post partum. Implementasi yang dilakukan telah sesuai dengan perencanaan yang ditetapkan namun belum mendokumentasikan secara terperinci. Evaluasi telah didokumentasikan menggunakan catatan perkembangan pasien terintegrasi yang terdiri atas SOAP namun terdapat sedikit kesenjangan dengan teori yang peneliti gunakan sebagai acuan yaitu pada bagian assessment.
