Abstract

NURSING CARE INSTALLABILITY OF HYPERGLYCEMIC BLOOD GLUCOSE LEVELS IN PRE AND POST DIABETIC RETINOPATHY PATIENTS VITRECTOMY WITH INTERVENTION REFLECTION MASSAGE THERAPY IN BALI MANDARA EYE Hospital 2022 ABSTRACT Diabetic retinopathy (DR) is an eye disorder in patients with diabetes mellitus caused by damage to the micro blood vessels (capillaries) of the retina in various degrees so that it interferes with vision ranging from mild to severe even to permanent blindness and this is associated with high blood sugar levels. hyperglycemia) over time. Reflexology massage is a form of complementary therapy by stimulating the foot area to stimulate the reflex point by using the fingers or tools such as wood, plastic or rubber. The purpose of the nursing care description of the instability of blood glucose levels in Diabetic Retinopathy patients at the Bali Mandara Eye Hospital in 2022. This study chose a descriptive type of research with a case study design. The number and size of the sample in the preparation of this case study is one patient with a nursing problem of glucose level instability. The results of the study showed that the patient's assessment data complained of lethargy, dry mouth and feeling thirsty, the patient had a history of type II diabetes mellitus, laboratory examinations showed that the patient's HbA1c was 7.1% and the patient's blood sugar was 381 mg/dL. The nursing diagnosis formulated was the instability of blood glucose levels associated with insulin resistance as evidenced by the patient saying lethargic, said his mouth was dry and very thirsty, the result of GDS was 381 mg/dL. The nursing plan refers to the SLKI guidelines with the outcome of increased blood sugar stability and SIKI with the main intervention for hyperglycemia management, as well as reflexology therapy innovation interventions. Nursing implementation is carried out according to the planned nursing plan for 3x24 hours. The nursing evaluation showed that the goals were achieved as evidenced by the patient's decreased lethargy, decreased dry mouth, decreased thirst, objective data: the patient looked better, GDS 198 mg/dL. Reflexology therapy intervention can be used as an intervention in dealing with the problem of unstable blood sugar levels in diabetic retinopathy patients. ASUHAN KEPERAWATAN KETIDAKSTABILAN KADAR GLUKOSA DARAH HIPERGLIKEMI PADA PASIEN RETINOPATI DIABETIK PRE DAN POST OPERASI VITREKTOMI DENGAN INTERVENSI TERAPI PIJAT REFLEKSI DI RS MATA BALI MANDARA TAHUN 2022 ABSTRAK Retinopati Diabetik (DR) adalah kelainan mata pada pasien diabetes melitus yang disebabkan oleh kerusakan pembuluh darah mikro (kapiler) retina dalam berbagai tingkatan sehingga mengganggu penglihatan mulai dari yang ringan sampai berat bahkan sampai mejadi kebutaan permanen dan hal ini dikaitkan dengan kadar gula yang tinggi (hiperglikemi) secara berkepanjangan. Piajt refleksi merupakan salah satu bentuk dari terapi komplementer dengan melakukan rangsangan berupa penekanan pada daerah telapak kaki untuk merangsang titik reflex dengan menggunakan jari – jari tangan atau alat bantu seperti kayu, plastik maupun karet. Tujuan gambaran asuhan keperawatan ketidakstabilan kadar glukosa darah pada pasien Retinopati Diabetik di RS Mata Bali Mandara tahun 2022. Penelitian ini memilih jenis penelitian deskriptif dengan desain studi kasus. Jumlah dan besar sampel dalam penyusunan studi kasus ini adalah satu pasien dengan masalah keperawatan ketidakstabilan kadar glukosa. Hasil penelitian didapatkan data pengkajian pasien mengeluh lesu, mulut kering dan merasa haus, pasien memiliki riwayat penyakit diabetes mellitus tipe II, pemeriksaan laboratorium diperoleh hasil HbA1c pasien sebesar 7,1% dan gula darah sewaktu pasien sebesar 381 mg/dL. Diagnosis keperawatan yang dirumuskan adalah ketidakstabilan kadar glukosa darah berhubungan dengan resistensi insulin dibuktikan dengan pasien mengatakan lesu, mengatakan mulutnya kering dan sangat haus, hasil gds 381 mg/dL. Rencana keperawatan mengacu pada pedoman SLKI dengan luaran kestabilan kadar gula darah meningkat dan SIKI dengan intervensi utama manajemen hiperglikemia, serta intervensi inovasi terapi pijat refleksi. Implementasi keperawatan dilakukan sesuai dengan rencana keperawatan yang sudah direncanakan selama 3x24 jam. Evaluasi keperawatan menunjukkan tujuan tercapai dibuktikan dengan pasien lesu menurun, mulut kering menurun, rasa haus menurun, data objektif: pasien tampak lebih baik, GDS 198 mg/dL. Intervensi terapi pijat refleksi dapat digunakan sebagai intervensi dalam menangani masalah ketidakstabilan kadar gula darah pada pasien retinopati diabetik.
