Abstract

THE EFFECT OF HAND MASSAGE BY FAMILY RELATIVES ON PAIN INTENSITY IN CANCER PATIENTS AT BALI MANDARA REGIONAL HOSPITAL IN 2026 ABSTRACT Pain in cancer patients is triggered by the release of proinflammatory cytokines by M1 macrophages and mechanical pressure of the tumor mass in the microenvironment. These ligands bind to nociceptors, activate the TAC1 gene to release the neuropeptide Substance P, and trigger neurogenic inflammation and peripheral sensitization that causes nerve hypersensitivity. Pain signals are transmitted through A-delta and C fibers to the dorsal horn of the spinal cord to be transmitted to the brain. Complementary therapy such as hand massage by close family members is needed to help block the transmission of pain signals based on the gate control theory. This study aims to determine the effect of hand massage by close family members on pain intensity in cancer patients. A quantitative pre-experimental study with a one-group pretest-posttest design was conducted on 23 respondents using a consecutive sampling technique. The intervention was given for 10 minutes twice a day for 3 days. Pain intensity was measured using a Numeric Rating Scale and analyzed using a Paired Samples t-test. The majority of respondents were female (65.2%), with an average age of 54.87 years, suffering from breast cancer (43.5%), stage III (56.5%), and received massages from children (56.5%). The average pain intensity decreased from 7.08 ± 2.043 to 3.34 ± 1.465 with a p value of 0.000 (p <0.05), indicating a significant effect of hand massage by the closest family on reducing pain intensity in cancer patients. It is recommended that hand massage be integrated as a non-pharmacological therapy through education and family involvement. Ligan tersebut berikatan dengan nosiseptor, mengaktivasi gen TAC1 untuk melepaskan neuropeptida Substansi P, serta memicu inflamasi neurogenik dan sensitisasi perifer yang menyebabkan hipersensitivitas saraf. Sinyal nyeri dihantarkan melalui serabut A-delta dan C menuju kornu dorsalis medula spinalis untuk diteruskan ke otak. Terapi pendamping seperti hand massage oleh keluarga terdekat diperlukan untuk membantu memblokir transmisi sinyal nyeri berdasarkan teori gate control. Penelitian ini bertujuan mengetahui pengaruh hand massage oleh keluarga terdekat terhadap intensitas nyeri pada pasien kanker. Penelitian kuantitatif pre-experimental dengan rancangan one group pretest-posttest dilakukan pada 23 responden menggunakan teknik consecutive sampling. Intervensi diberikan selama 10 menit dilakukan 2 kali sehari dalam 3 hari. Intensitas nyeri diukur menggunakan Numeric Rating Scale dan dianalisis dengan uji Paired Samples t-test. Mayoritas responden perempuan (65,2%), usia rata-rata 54,87 tahun, menderita ca mammae (43,5%), stadium III (56,5%), dan dipijat oleh anak (56,5%). Rata-rata intensitas nyeri menurun dari 7,08 ± 2,043 menjadi 3,34 ± 1,465 dengan nilai p=0,000 (p<0,05), sehingga terdapat pengaruh signifikan pemberian hand massage oleh keluarga terdekat terhadap penurunan intensitas nyeri pada pasien kanker. Disarankan agar hand massage diintegrasikan sebagai terapi nonfarmakologis melalui edukasi dan pelibatan keluarga. Kata kunci : hand massage; keluarga; kanker; nyeri
