Abstract

NURSING CARE OF MR. R WITH INEFFECTIVE AIRWAY CLEARANCE DUE TO CHRONIC OBSTRUCTIVE PULMONARY DISEASE (COPD) IN CEMPAKA 3 WARD OF TABANAN REGIONAL GENERAL HOSPITAL IN 2026 ABSTRACT Chronic Obstructive Pulmonary Disease (COPD) is a progressive chronic disease characterized by airflow limitation and increased sputum production, leading to ineffective airway clearance. This case report aims to describe the nursing care of Mr. R with ineffective airway clearance due to COPD in Cempaka 3 Ward of Tabanan Regional General Hospital in 2026. The design of this case report used a descriptive method in the form of a case study with a nursing process approach, including assessment, diagnosis, planning, implementation, and evaluation conducted over 4×24 hours. The assessment results showed that the patient experienced ineffective coughing, excessive sputum production, rhonchi, dyspnea, difficulty speaking, orthopnea, restlessness, as well as changes in respiratory rate (26 breaths/minute) and breathing pattern (rapid and shallow). The nursing diagnosis formulated was ineffective airway clearance related to retained secretions. The main interventions included effective coughing exercises, airway management, and respiratory monitoring, with supportive intervention in the form of inhalation therapy. The evaluation results showed improvement in coughing effectiveness, reduced sputum production, decreased rhonchi, reduced dyspnea, no difficulty speaking, ability to lie comfortably without shortness of breath, reduced restlessness, and improved respiratory rate (20 breaths/minute) and breathing pattern. R DENGAN BERSIHAN JALAN NAPAS TIDAK EFEKTIF AKIBAT PENYAKIT PARU OBSTRUKTIF KRONIS (PPOK) DI RUANG CEMPAKA 3 RSUD TABANAN TAHUN 2026 ABSTRAK Penyakit Paru Obstruktif Kronis (PPOK) merupakan penyakit kronis progresif yang ditandai dengan keterbatasan aliran udara dan peningkatan produksi sputum, sehingga dapat menimbulkan masalah keperawatan bersihan jalan napas tidak efektif. Laporan kasus ini bertujuan untuk mengetahui asuhan keperawatan Tn. R dengan bersihan jalan napas tidak efektif akibat PPOK di Ruang Cempaka 3 RSUD Tabanan tahun 2026. Desain laporan kasus yang digunakan adalah metode deskriptif berupa studi kasus dengan pendekatan proses keperawatan yang meliputi pengkajian, diagnosis, perencanaan, implementasi, dan evaluasi selama 4×24 jam. Hasil pengkajian menunjukkan pasien mengalami batuk tidak efektif, sputum berlebih, ronkhi, dispnea, sulit bicara, ortopnea, gelisah, serta perubahan frekuensi (26x/menit) dan pola napas (cepat/dangkal). Diagnosis keperawatan yang telah dirumuskan adalah bersihan jalan napas tidak efektif berhubungan dengan sekresi yang tertahan. Intervensi utama berupa latihan batuk efektif, manajemen jalan napas, dan pemantauan respirasi dengan intervensi pendukung berupa pemberian obat inhalasi. Hasil evaluasi menunjukkan adanya perbaikan kondisi pasien berupa peningkatan kemampuan batuk efektif, penurunan sputum, ronkhi terdengar menurun, sesak napas berkurang, sudah tidak sulit bicara, dapat berbaring dengan nyaman tanpa mengalami sesak napas, gelisah menurun, serta membaiknya frekuensi (20x/menit) dan pola napas.
