Abstract
NURSING CARE FOR MR.M WITH HYPERVOLEMIA DUE TO CHRONIC KIDNEY DISEASE (CKD) IN THE JUMPAI ROOM OF KLUNGKUNG REGIONAL HOSPITAL IN 2026 ABSTRACT Chronic Kidney Disease (CKD) is a progressive kidney dysfunction that causes fluid and electrolyte imbalance. Hypervolemia is characterized by an increase in body fluid volume that can cause symptoms such as edema and respiratory distress. The purpose of this case report is to determine the implementation of nursing care in patients with hypervolemia due to chronic kidney disease (CKD). The method used is a case study with a nursing process approach that includes assessment, diagnosis, planning, implementation, and evaluation. The subject in this case report is Mr. M, a male, 60 years old, with a medical diagnosis of CKD Stage 5. The results of the assessment showed that Mr. M experienced shortness of breath, orthopnea, proximal nocturnal dyspnea, edema in the extremities, JVP 10cmH2O, positive hepatojugular reflex, jugular venous distension, additional breath sounds (ronchi), and oliguria (300 cc/24 hours). The established nursing diagnosis is hypervolemia related to impaired regulatory mechanisms. Interventions provided included hypervolemia management, fluid monitoring, and vital signs monitoring for 4x24 hours. Evaluation results showed improvement in the patient's condition, indicated by decreased shortness of breath, reduced edema, and improved fluid balance, thus achieving nursing goals. It is hoped that health workers can improve monitoring of fluid balance and hemodynamic conditions of patients optimally to prevent complications of hypervolemia in Chronic Kidney Disease patients. Keywords: Chronic kidney disease, hypervolemia, nursing care ASUHAN KEPERAWATAN TN.M DENGAN HIPERVOLEMIA AKIBAT CHRONIC KIDNEY DISEASE (CKD) DI RUANG JUMPAI RSUD KLUNGKUNG TAHUN 2026 ABSTRAK Penyakit Ginjal Kronis (PGK) merupakan gangguan fungsi ginjal yang bersifat progresif dan menyebabkan ketidakseimbangan cairan dan elektrolit. Hipervolemia ditandai dengan peningkatan volume cairan tubuh yang dapat menimbulkan gejala seperti edema dan gangguan pernapasan. Tujuan laporan kasus ini adalah untuk mengetahui pelaksanaan asuhan keperawatan pada pasien dengan hipervolemia akibat chronic kidney disease (CKD). Metode yang digunakan adalah studi kasus dengan pendekatan proses keperawatan yang meliputi pengkajian, diagnosis, perencanaan, implementasi, dan evaluasi. Subjek dalam laporan kasus ini adalah Tn.M, laki-laki, usia 60 tahun dengan diagnosis medis CKD Stadium 5. Hasil pengkajian menunjukkan Tn.M mengalami sesak napas, ortopnea, proxymal noctural dipsnea, edema pada ekstermitas, JVP 10cmH2O, refleks hepatojugular positif, distensi vena jugularis, suara napas tambahan (ronchi), serta oliguria (300 cc/24 jam). Diagnosis keperawatan yang ditegakkan adalah hipervolemia berhubungan dengan gangguan mekanisme regulasi. Intervensi yang diberikan meliputi manajemen hipervolemia, pemantauan cairan dan pemantauan tanda-tanda vital selama 4x24 jam. Hasil evaluasi menunjukkan kondisi pasien membaik ditandai dengan penurunan sesak napas, edema berkurang, serta keseimbangan cairan membaik, sehingga tujuan keperawatan tercapai. Diharapkan tenaga kesehatan dapat meningkatkan pemantauan keseimbangan cairan dan kondisi hemodinamik pasien secara optimal untuk mencegah komplikasi hipervolemia pada pasien Chronic Kidney Disease.
