Abstract

NURSING CARE FOR PATIENTS WITH INEFFECTIVE AIRWAY CLEARANCE DUE TO CHRONIC OBSTRUCTIVE PULMONARY DISEASE IN OLEG ROOM OF MANGUSADA HOSPITAL ABSTRACT Chronic Obstructive Pulmonary Disease (COPD) is a progressive and chronic disorder of the lungs caused by excessive mucus production or hypersecretion, as well as long-term exposure to irritants, such as cigarette smoke, and is characterized by shortness of breath, cough, and wheezing. This condition is common in both active and passive smokers. Clinical symptoms of COPD are often related to nursing problems such as ineffective airway clearance. This case report discusses the nursing care provided to Mrs. S, a COPD patient treated in the Oleg Room, Mangusada Hospital, from April 2 to April 6, 2025. Mrs. S complained of shortness of breath for the past four days. Sputum analysis and sputum culture examinations were positive for respiratory distress due to secretions accumulating in the airways. The nursing diagnosis was established as ineffective airway clearance related to airway hypersecretion, evidenced by ineffective cough, excessive sputum, and wheezing. The interventions provided include effective coughing exercises, airway management, and respiratory monitoring. After five days of treatment, the patient's condition improved and the nursing goals were achieved. It was concluded that nursing care plays an important role in addressing ineffective airway problems in COPD. Collaboration with the medical team and further education is necessary to maintain results through treatment and non-pharmacological interventions such as effective coughing exercises and chest physiotherapy. Keywords: COPD, ineffective airway clearance, nursing care, airway ASUHAN KEPERAWATAN PADA PASIEN DENGAN BERSIHAN JALAN NAFAS TIDAK EFEKTIF AKIBAT PENYAKIT PARU OBSTRUKTIF KRONIK DI RUANG OLEG RSD MANGUSADA ABSTRAK Penyakit Paru Obstruktif Kronik (PPOK) adalah gangguan progresif dan kronis pada paru-paru yang disebabkan kelebihan produksi lendir atau hipersekresi serta oleh paparan iritan jangka panjang, seperti asap rokok, dan ditandai oleh sesak napas, batuk, dan ronkhi. Kondisi ini umum terjadi pada perokok aktif maupun pasif. Gejala klinis PPOK seringkali berkaitan dengan masalah keperawatan seperti bersihan jalan napas tidak efektif. Laporan kasus ini membahas asuhan keperawatan pada Ny. S, pasien PPOK yang dirawat di Ruang Oleg, RSD Mangusada, dari 2–6 April 2025. Ny. S mengeluhkan sesak napas sejak empat hari sebelumnya. Pemeriksaan analisis sputum dan kultur sputum pasien positif mengalami gangguan pernafasan karena sekresi yang menumpuk dijalan nafas. Diagnosis keperawatan ditegakkan sebagai bersihan jalan napas tidak efektif berhubungan dengan hipersekresi jalan nafas, dibuktikan dengan batuk tidak efektif, sputum berlebih, dan ronkhi. Intervensi yang diberikan meliputi latihan batuk efektif, manajemen jalan napas, dan pemantauan respirasi. Setelah lima hari perawatan, kondisi pasien membaik dan tujuan keperawatan tercapai. Disimpulkan bahwa asuhan keperawatan berperan penting dalam menangani masalah jalan napas tidak efektif pada PPOK. Diperlukan kolaborasi dengan tim medis dan edukasi lanjutan untuk mempertahankan hasil melalui pengobatan dan penanganan non-farmakologis seperti latihan batuk efektif dan fisioterapi dada.
