Abstract

NURSING CARE FOR Mrs. W WITH BLOOD GLUCOSE LEVEL INSTABILITY DUE TO TYPE 2 DIABETES MELLITUS IN MR. P'S FAMILY (Case Report in the Working Area of UPTD Puskesmas IV, South Denpasar District Health Office in 2025) ABSTRACT Blood glucose instability is a condition in which blood sugar levels increase or decrease outside the normal range. Globally, it is estimated that in 2025, the number of people with diabetes mellitus will increase to 1 billion to 31 billion people. This case report aims to determine nursing care for patients with blood glucose instability in the work area of Puskesmas IV, South Denpasar. This case report is Mrs. W, a patient with type 2 diabetes mellitus who meets the inclusion criteria. This case report is presented descriptively. The results of the assessment found that the patient's main complaints were feeling tired quickly, blood glucose levels increased to 277 mg / dL, increased thirst, and dry mouth. The nursing diagnosis identified was blood glucose instability (D.0027) due to insulin resistance. The interventions identified were hyperglycemia management and physical exercise education with diabetes stretching once a day for 20-30 minutes. After nursing care was carried out for 5 days in the morning for 60 minutes, the stability of blood glucose levels improved with the results of decreased fatigue, decreased thirst, decreased dry mouth, and blood glucose levels improved to 184 mg / dL. In conclusion, it shows that this nursing care is effective to apply. The suggestion that can be given is that this nursing care can be an intervention in the management of hyperglycemia with hyperglycemia management and physical exercise education. P (Laporan Kasus di Wilayah Kerja UPTD Puskesmas IV Dinas Kesehatan Kecamatan Denpasar Selatan Tahun 2025) ABSTRAK Ketidakstabilan kadar glukosa darah merupakan kondisi di mana kadar gula darah mengalami kenaikan atau penurunan di luar rentang normal. Kondisi ini terjadi akibat resistensi insulin yang menyebabkan penyakit diabetes mellitus. Secara global, jumlah penderita diabetes mellitus diperkirakan pada tahun 2025, akan meningkat hingga 1 miliar hingga 31 miliar orang. Laporan kasus ini bertujuan untuk mengetahui asuhan keperawatan pada pasien dengan ketidakstabilan kadar glukosa darah di wilayah kerja puskesmas IV Denpasar Selatan. Laporan kasus ini adalah Ny.W penderita diabetes mellitus tipe 2 yang memenuhi kriteria inklusi. Laporan kasus ini disajikan secara deskriptif. Hasil pengkajian ditemukan keluhan utama pasien merasa cepat lelah, kadar glukosa darah meningkat menjadi 277mg/dL, haus meningkat, dan mulut kering. Diagnosis keperawatan yang diidentifikasi adalah ketidakstabilan kadar glukosa darah (D.0027) akibat resistensi insulin. Intervensi yang diidentifikasi adalah manajemen hiperglikemia dan edukasi latihan fisik dengan streching diabetes sehari sekali selama 20-30 menit. Setelah dilakukan asuhan keperawatan selama 5 hari pada pagi hari selama 60 menit kestabilan kadar glukosa darah membaik dengan hasil lelah menurun, haus menurun, mulut kering menurun, dan kadar glukosa darah membaik menjadi 184mg/dL. Kesimpulannya, menunjukkan bahwa asuhan keperawatan ini efektif untuk diterapkan. Saran yang dapat diberikan adalah diharapkan asuhan keperawatan ini dapat menjadi intervensi dalam pengelolaan hiperglikemia dengan manajemen hiperglikemia dan edukasi latihan fisik.
