Abstract

NURSING CARE AT MR. M WITH INEFFECTIVE AIRWAY CLEARANCE DUE TO COPD IN THE PIKAT ROOM OF THE HOSPITAL KLUNGKUNG YEARS 2025 ABSTRACT Ineffective airway cleaning is the inability to clear secretions or obstructions of the airway to maintain the airway patent (PPNI SDKI DPP TIM Working Group 2017). In seeking secretion expenditure, ineffective airway cleaning is suitable to be a priority problem in COPD patients (Budhi, et al. 2023). According to the Indonesian Nursing Intervention Standards to overcome the problem of ineffective airway clearance, there are several main interventions, namely effective cough exercises, airway management, respiratory monitoring, administration of inhaled drugs. (PPNI DPP SIKI 2018). Nursing care case reports in patients with ineffective airway clearance due to Chronic Obstructive Pulmonary Disease (COPD) with a nursing care approach. The sample in this case report is a patient with COPD who meets the inclusion criteria, who is treated for five days. This case report is presented descriptively to describe the process of nursing as a whole. The results of the study were found in the form of patients complaining of shortness of breath and difficulty speaking, patients also seemed to have difficulty coughing or being unable to cough, coughing up phlegm, difficulty expelling phlegm, and the respiratory sound of the patient sounding wheezing. The nursing diagnosis formulated was ineffective airway clearance due to COPD (D.0001). After five days of treatment, the patient said that shortness of breath had decreased, difficulty in speaking had decreased, effective cough had increased, sputum production had decreased, wheezing breathing sound had decreased, anxiety had decreased. The results of the evaluation showed that the problem of airway clearance had been resolved by planning interventions that were stopped and maintaining the patient's condition. From the case report, it was concluded that nursing care can help in overcoming airway clearance in COPD patients. Keywords: Ineffective Airway Cleaning, COPD ASUHAN KEPERAWATAN PADA TN. M DENGAN BERSIHAN JALAN NAPAS TIDAK EFEKTIF AKIBAT PPOK DI RUANG PIKAT RSUD KLUNGKUNG TAHUN 2025 ABSTRAK Bersihan jalan napas tidak efektif adalah ketidakmampuan untuk membersihkan sekret atau obstruksi jalan napas untuk mempertahankan jalan napas tetap paten (PPNI SDKI DPP TIM Pokja 2017). Dalam mengupayakan pengeluaran sekret, bersihan jalan napas tidak efektif cocok untuk menjadi prioritas masalah pada pasien PPOK (Budhi, dkk 2023). Menurut Standar Intervensi Keperawatan Indonesia untuk mengatasi masalah bersihan jalan nafas tidak efektif terdapat beberapa intervensi utama yaitu Latihan batuk efektif, manajemen jalan napas , pemantauan respirasi pemberian obat inhalasi. (PPNI DPP SIKI 2018). Laporan kasus asuhan keperawatan pada pasien bersihan jalan napas tidak efektif akibat Penyakit Paru Obtruktif Kronis (PPOK) dengan pendekatan asuhan keperawatan. Sample pada laporan kasus ini merupakan pasien penderita PPOK yang memenuhi kriteria inklusi, yang dirawat selama lima hari. Laporan kasus ini disajikan secara deskriptif untuk menggambarkan proses dari keperawatan secara utuh. Hasil pengkajian yang ditemukan berupa pasien mengeluh sesak nafas dan sulit berbicara, pasien juga tampak kesulitan untuk batuk atau tidak mampu batuk, batuk berdahak, kesulitan untuk mengeluarkan dahak, suara pernapasan dari pasien berbunyi wheezing. Diagnosis keperawatan yang dirumuskan adalah bersihan jalan napas tidak efektif akibat PPOK (D.0001). Setelah dilakukan perawatan lima hari, Pasien mengatakan bahwa sesak napas sudah menurun, kesulitan dalam berbicara sudah menurun, batuk efektif meningkat, produksi sputum menurun,bunyi napas wheezing menurun, gelisah menurun. Hasil evaluasi menunjukan masalah bersihan jalan napas sudah teratasi dengan perencanaan intervensi yang dihentikan serta mempertahankan kondisi pasien. Dari laporan kasus tersebut disimpulkan bahwa asuhan keperawatan dapat membantu dalam mengatasi bersihan jalan napas pada pasien PPOK. Kata kunci : Bersihan Jalan Napas Tidak Efektif, PPOK
