Abstract

NURSING CARE FOR INEFFECTIVE AIRWAY CLEARANCE WITH BUTEYKO BREATHING TECHNIQUE THERAPY IN COPD PATIENTS AT CEMPAKA ROOM GENERAL BANGLI HOSPITAL. COPD is a chronic obstructive respiratory disease that is irreversible. Excessive mucus secretion results in ineffective airway clearance. The aim of this scientific paper is to provide an overview of nursing care for ineffective airway clearance with Buteyko breathing technique therapy in COPD patients. Assessment; found 7 symptoms and signs that support the nursing problem of ineffective airway clearance, namely ineffective cough, excessive sputum, dry rhonchi heard on lung auscultation, dyspnea (SpO2 95%), restlessness, altered breathing pattern and altered breathing frequency. Nursing interventions; airway management, effective coughing exercises, respiratory monitoring and Buteyko breathing technique innovation therapy twice a day for 15-20 minutes per exercise session with an oxygen saturation indicator of 95-98%. Nursing Implementation provided; chest physiotherapy, providing a semi-Fowler position, providing effective coughing exercises, providing oxygen 3 lpm, providing nebulized lasacom 2.5 mg and budesma 0.5 mg every 8 hours, providing mucolytic resfar 3x600 mg (IV) according to the doctor's collaboration and providing Buteyko breathing technique exercises according to SOP. Nursing evaluation results; ineffective airway clearance resolved. Conclusion; Buteyko breathing technique therapy is effective in overcoming hyperventilation, maintaining stable oxygen saturation (98%), overcoming respiratory muscle fatigue, reducing mucus production thereby increasing airway clearance in patients with COPD. Buteyko breathing technique can be used as an alternative non-pharmacological therapy option in providing independent intervention for nurses in COPD patients with ineffective airway clearance. Keywords: Ineffective Airway Clearance, COPD, Buteyko Breathing Technique ASUHAN KEPERAWATAN BERSIHAN JALAN NAFAS TIDAK EFEKTIF DENGAN PEMBERIAN TERAPI BUTEYKO BREATHING TECHNIQUE PADA PASIEN PPOK DI RUANG CEMPAKA RSUD BANGLI. PPOK adalah penyakit pernapasan obstruktif kronis yang bersifat ireversibel. Sekresi mucus berlebih mengakibatkan bersihan jalan napas tidak efektif. Tujuan karya tulis ilmiah ini adalah untuk mengetahui gambaran asuhan keperawatan bersihan jalan napas tidak efektif dengan terapi buteyko breathing technique pada pasien PPOK. Pengkajian; ditemuan 7 gejala dan tanda yang mendukung masalah keperawatan bersihan jalan napas tidak efektif yaitu batuk tidak efektif, sputum berlebih, auskultasi paru terdengar ronkhi kering, dispnea (SpO2 95%), gelisah, pola napas berubah dan frekuensi napas berubah. Intervensi keperawatan yang diberikan; manajemen jalan napas, latihan batuk efektif, pemantauan respirasi dan terapi inovasi buteyko breathing technique 2 kali sehari durasi 15-20 menit per sesi latihan dengan indikator saturasi oksigen 95-98%. Implementasi keperawatan yang dilakukan; fisioterapi dada, memberikan posisi semi fowler, memberikan latihan batuk efektif, memberikan oksigen 3 lpm, memberikan nebulasi lasacom 2,5 mg dan budesma 0,5 mg setiap 8 jam, memberikan mukolitik resfar 3x600 mg (IV) sesuai kolaborasi dokter dan memberikan latihan buteyko breathing technique sesuai SOP. Hasil evaluasi keperawatan; bersihan jalan napas tidak efektif teratasi. Simpulan; terapi buteyko breathing technique efektif mengatasi hiperventilasi, menjaga kestabilan saturasi oksigen (98%), mengatasi kelelahan otot pernafasan, menurunkan produksi mukus sehingga meningkatkan bersihan jalan napas pada pasien dengan PPOK. Buteyko breathing technique dapat dijadikan alternatif pilihan terapi non farmakologi dalam memberikan intervensi mandiri bagi perawat pada pasien PPOK dengan bersihan jalan napas tidak efektif.
