Abstract

NURSING CARE PLAN IN ENGLISH FOR Mrs,M WHO IS AT RISK OF INFECTION DUE TO AN EPISIOTOMY WOUND AT PUSKESMAS IV DENPASAR SELATAN ABSTRACT In general, every expectant mother hopes for a smooth and uncomplicated delivery process. However, trauma to the perineum often occurs during childbirth. The increasing number of normal deliveries over the past three years at Puskesmas IV South Denpasar has contributed to a higher risk of infection in postpartum primigravida mothers due to birth canal injuries. This study aims to describe the nursing care provided to postpartum mothers at risk of perineal wound infection. This research uses a descriptive method with a case report approach. Data were obtained through observation and documentation study of two postpartum patients who had normal deliveries and were treated at Puskesmas IV South Denpasar. The results of the study showed that the assessment data obtained in the field aligned with the theoretical framework used as a reference. The nursing diagnosis applied at Puskesmas IV South Denpasar was "risk of infection," based on the standardized nursing diagnosis (SDKI). The care plan implemented included perineal care and vulvar hygiene, and patient records included documented nursing care plans. The implementation of nursing interventions followed the established plan, and evaluation was documented through patient progress notes integrated in the SOAP format, based on the assessment theory used as a reference. Keywords: Nursing care, risk of infection, episiotomy wound ASUHAN KEPERAWATAN PADA Ny.M DENGAN RISIKO INFEKSI AKIBAT LUKA EPISIOTOMI DI DENPASAR SELATAN ABSTRAK Secara umum, setiap calon ibu menginginkan proses persalinan yang berlangsung lancar dan tanpa kendala. Namun, sering kali selama persalinan terjadi trauma pada perineum. Peningkatan jumlah persalinan normal selama tiga tahun terakhir di Puskemas IV Denpasar Selatan turut berkontribusi terhadap meningkatnya risiko infeksi pada ibu post partum primigravida akibat luka pada jalan lahir. Penelitian ini bertujuan untuk menggambarkan asuhan keperawatan pada ibu post partum yang berisiko mengalami infeksi luka perineum. Penelitian ini menggunakan metode deskriptif dengan pendekatan Laporan kasus. Data diperoleh melalui observasi dan studi dokumentasi terhadap dua pasien post partum normal yang dirawat di Puskesmas IV Denpasar Selatan Hasil penelitian menunjukkan bahwa data pengkajian yang diperoleh di lapangan sesuai dengan teori yang digunakan sebagai acuan. Diagnosis keperawatan yang digunakan di Puskesmas IV Denpasar Selatan adalah resiko infeksi sesuai dengan teori standar SDKI. perencanaan yang diterapkan yaitu perawatan perineum vulva hygiene, dokumen pasien mencantumkan perencanaan keperawatan. Pelaksanaan tindakan telah sesuai dengan perencanaan, Evaluasi dicatat melalui catatan perkembangan pasien terintegrasi dalam format SOAP, teori yang dijadikan acuan, khususnya pada bagian assessment.
