Abstract

NURSING CARE FOR INEFFECTIVE AIRWAY CLEARANCE WITH WATER INHALATION AND EUCALYPTUS OIL IN PULMONARY TUBERCULOSIS PATIENTS AT RSUD TABANAN DISTRICT ABSTRACT Chronic irritation in tuberculosis causes goblet cell proliferation which results in mucus hypersecretion, causing ineffective airway clearance nursing problems. The purpose of this study was to describe and analyze nursing care for ineffective airway clearance with water inhalation with eucalyptus oil in pulmonary tuberculosis at Tabanan Hospital using descriptive analysis method. The subject of the study was a patient with a diagnosis of pulmonary tuberculosis of one person with the problem of ineffective airway clearance. The nursing diagnosis formulated was ineffective airway clearance associated with retained secretions. Planned interventions are airway management, effective cough training, monitoring respiration and innovative water inhalation with eucalyptus oil which is done twice a day in the morning and evening for 10-15 minutes. Evaluation results after intervening for 3x24 hours obtained effective cough increased, sputum production decreased, wheezing decreased, dipsnea decreased, breathing frequency improved, breathing patterns improved. The conclusion of this study is that the provision of water and eucalyptus oil inhalation therapy can improve airway clearance in TB patients and can be used by nurses as an alternative complementary-based action to solve the problem of ineffective airway clearance. Tujuan dari penelitian ini adalah mendeskripsikan dan menganalisis asuhan keperawatan bersihan jalan napas tidak efektif dengan inhalasi uap air dengan minyak kayu putih pada tuberkulosis paru di RSUD Tabanan dengan menggunakan metode analisis deskriptif. Subjek penelitian adalah pasien dengan diagnosa tuberkulosis paru sejumlah satu orang dengan masalah bersihan jalan napas tidak efektif. Diagnosis keperawatan yang dirumuskan ialah bersihan jalan napas tidak efektif berhubungan dengan sekresi yang tertahan. Intervensi yang direncanakan adalah manajemen jalan napas, Latihan batuk efektif, pemantauan respirasi dan inovasi inhalasi uap air dengan minyak kayu putih yang dilakukan sebanyak 2x sehari saat pagi dan sore selama 10-15 menit. Hasil evaluasi setelah melakukan intervensi selama 3x24 jam yang didapatkan batuk efektif meningkat, produksi sputum menurun, wheezing menurun, dipsnea menurun, frekuensi napas membaik, pola napas membaik. Simpulan dari penelitian ini, pemberian terapi inhalasi uap air dan minyak kayu putih dapat meningkatkan bersihan jalan napas pada pasien TBC dan dapat digunakan perawat sebagai tindakan alternatif berbasis komplementer pemecahan masalah bersihan jalan napas tidak efektif.
