Abstract

IMPLEMENTATION OF CHEST PHYSIOTHERAPY TO PATIENTS WITH INEFFECTIVE AIRWAY CLEANING DUE TO PNEUMONIA AT MANGUSADA BADUNG HOSPITAL IN 2024 ABSTRACT Pneumonia is an infection of the distal lower respiratory tract, especially the alveolar spaces, including the small bronchi and bronchioles, which has general symptoms in the form of shortness of breath, cough with phlegm, additional breath sounds and increased body temperature or fever. Pneumonia is currently listed as a health problem in adults. Pneumonia sufferers increase with age. In the 55-64 year age group it reached 2.5% and in the 65-74 year age group it was 3.0% (RI Ministry of Health 2022). People who experience pneumonia can result in reduced sensitivity of the respiratory center to hypoxia or hypercapnia in elderly patients resulting in loss of ventilatory response. This study aims to describe nursing care for pneumonia patients with ineffective airway clearance problems who are given chest physiotherapy in the Oleg room at Mangusada RSD. The method used for data collection in this case study is the interview and observation method. The results of this case study show that effective coughing has increased, sputum production has decreased, crackles have decreased, dyspnea has decreased, respiratory frequency has improved, and breathing patterns have improved. The conclusion obtained after taking action on the subject, starting from assessment to nursing evaluation, was that the implementation of chest physiotherapy in pneumonia patients can expel phlegm so that the patient's breathing pattern becomes effective again, they can breathe freely and the body gets enough oxygen, and provides a feeling of comfort. Keywords: Pneumonia, Ineffective airway clearance, Chest physiotherapy IMPLEMENTASI PEMBERIAN FISIOTERAPI DADA PADA PASIEN DENGAN BERSIHAN JALAN NAFAS TIDAK EFEKTIF AKIBAT PNEUMONIA DI RSD MANGUSADA TAHUN 2024 ABSTRAK Pneumonia merupakan infeksi saluran pernafasan bagian bawah bagian distal, terutama ruang alveolar, termasuk bronkus kecil dan bronkiolus, memiliki gejala umum berupa sesak nafas, batuk berdahak, adanya suara napas tambahan dan peningkatan suhu tubuh atau demam. Pneumonia hingga saat ini tercatat sebagai masalah kesehatan pada orang-orang dewasa. Penderita pneumonia meningkat seiring dengan bertambahnya usia. Pada kelompok usia 55-64 tahun mencapai 2,5% dan pada kelompok usia 65-74 tahun sebesar 3,0% (Kemenkes RI 2022). Orang yang mengalami pneumonia dapat mengakibatkan berkurangnya sensitifitas dari pusat pernafasan terhadap hipoksia atau hiperkapnia pada pasien usia lanjut mengakibatkan hilangnya respon ventilasi. Penelitian ini bertujuan untuk mendeskripsikan asuhan keperawatan pada pasien pneumonia dengan masalah bersihan jalan napas tidak efektif yang diberikan fisioterapi dada di ruang oleg RSD Mangusada. Metode yang digunakan untuk pengumpulan data dalam studi kasus ini adalah metode wawancara dan observasi. Hasil studi kasus ini menunjukkan terdapat batuk efektif meningkat, produksi sputum menurun, ronki menurun, dispnea menurun, frekuensi napas membaik, pola napas membaik. Kesimpulan yang didapatkan setelah dilakukan tindakan kepada subjek mulai dari pengkajian sampai evaluasi keperawatan didapatkan bahwa implementasi fisoterapi dada pada pasien pneumonia dapat mengeluarkan dahak sehingga pola napas pasien menjadi efektif kembali, dapat bernapas dengan bebas dan tubuh mendapatkan oksigen yang cukup, dan memberikan rasa nyaman.
