Abstract

Congestive Heart Failure is a heart disease where the heart fails to pump to meet body’s need for adequete nutrients and oxygen One of nursing problem that often shows in congestive heart failure is intolerance of activity. Intolerance of activity can be defined as condition which a person does not have enough physiological or pshychological energy to complete daily activities. This study aims to determine the description of congestive heart failure with intolerance of activity. This reserach uses descriptive research with documentation data collection technique through two patient documents where the research is based on data attachment taken from Scientific Paper downloaded through Polythecnic Denpasar’s Repository Website. The result of the mayor and minor data on assessment are 60%. There are 67% of data examined in nursing diagnosa where the problem raised is intolerance of activity. Nursing planning consist of energy management and cardiacrehabilitation, only 38% is in the data. Implementation is based on nursing planning by 38% in the subject documents. Nursing evaluation used in the document is SOAP method, where there is a gap of 50%. This research expected to be used in monitoring the implementation of nursing care in patient with congestive heart failure with intolerance of activity. Keywords: Nursing Care, Intolerance of Activity, Congestive Heart Failure Gagal Jantung Kongestif adalah salah satu penyakit jantung dimana jantung mengalami kegagalan dalam memompa guna mencukupi kebutuhan sel-sel tubuh akan nutrien dan oksigen secara adekuat. Masalah yang paling sering muncul pada pasien gagal jantung kongestif adalah intoleransi aktivitas. Intoleransi aktivitas merupakan kondisi dimana seseorang tidak memiliki cukup energi fisiologis atau psikologis untuk menyelesaikan aktivitas sehari-hari Penelitian ini bertujuan untuk mengetahui gambaran asuhan keperawatan pada pasien gagal jantung kongestif dengan intoleransi aktivitas. Penelitian ini menggunakan jenis penelitian deskriptif dengan teknik pengumpulan data dokumentasi melalui dua dokumen pasien dimana penelitian berdasarkan lampiran data yang diambil melalui Karya Tulis Ilmiah yang diunduh melalui website repository Poltekkes Denpasar. Hasil pengkajian terdapat kesenjangan datar mayor dan minor sebesar 60%. Terdapat 67% data yang dikaji pada diagmosa keperawatan dimana masalah yang diangkat pada kedua subyek adalah intoleransi aktivitas. Perencanaan keperawatan terdiri dari manajemen energi dan rehabilitasi jantung dimana hanya 38% yang terdapat pada data dokumentasi. Implementasi yang dilakukan berdasarkan perencanaan keperawatan sebesae 38% pada dokumen subyek. Evaluasi keperawatan yang digunakan yaitu dengan metode dokumentasi SOAP, dimana terdapat kesenjangan sebesar 50%. Penelitian ini diharapkan dapat dijadikan sebagai bagan dalam melakukan monitoring pelaksanaan asuhan keperawatan pada pasien gagal jantung kongestif dengan intoleransi aktivitas.
