Abstract

Hipervolemia merupakan salah satu masalah keperawatan yang muncul pada pasien gagal jantung kongestif. Hipervolemia merupakan suatu kondisi peningkatan volume cairan intravaskular, interstisial, dan/atau intraselular. Tujuan dari penelitian ini adalah untuk mengetahui gambaran asuhan keperawatan pada pasien gagal jantung kongestif dengan masalah keperawatan hipervolemia di Ruang Cendrawasih RSUD Wangaya. Jenis penelitian yang digunakan dalam penelitian ini adalah penelitian deskriptif dengan rancangan studi kasus yang dilaksanakan dalam jangka waktu satu bulan yaitu pada bulan April sampai dengan bulan Mei 2019. Subjek dalam penelitian ini berjumlah dua orang yang sudah memenuhi kriteria inklusi dan ekslusi. Instrument yang digunakan dalam penelitian ini adalah pedoman observasi dokumentasi. Hasil yang telah diperoleh dari penelitian ini meliputi pengkajian, data subjektif dan objektif subjek sudah mengacu pada teori yang digunakan dalam penelitian ini. Diagnosa keperawatan, telah menggunakan format Problem, Etiology, Sign and Symptom (PES). Intervensi yang di rencanakan mengacu NIC NOC. Implementasi keperawatan telah dilaksanakan oleh perawat ruangan dan terdokumentasi pada catatan keperawatan subjek. Evaluasi keperawatan menggunakan format SOAP. Penelitian ini hendaknya dapat digunakan sebagai bahan pertimbangan dalam pendokumentasian catatan keperawatan pasien di ruangan dengan menggunakan pedoman diagnosa Standar Diagnosa Keperawatan Indonesia (SDKI). Kata kunci : Gagal Jantung Kongestif; Hipervolemia; Asuhan Keperawatan Hypervolaemia is one of the nursing problems that appear in patients with congestive heart failure. Hypervolaemia is a condition of increased intravascular, interstitial and/or intracellular fluid volume. The purpose of this study is to determine the description of nursing care in patients with congestive heart failure with hypervolemic nursing problems in the Cendrawasih Room of the Wangaya Hospital. The type of research used in this study is descriptive research with a case study design carried out within a period of one month, namely from April to May 2019. The subjects in this study amounted to two people who had met the inclusion and exclusion criteria. The instrument used in this study is a guideline for documentation observation. The results have been obtained from this study are, the subjective and objective data of the subject have referred to the theory that uses as a research guideline. The diagnoses have formulated in the format of Problem, Etiology, Sign, and Symptom (PES). The intervention planned refer to the NIC NOC. The implementation have implemented by the nurse and documented the subject's nursing records. The evaluation use SOAP format. This research should be used as a consideration in documenting the patient's nursing record in the room by referring to the Indonesian Nursing Diagnosis Standards guidelines.
