Abstract

Masa nifas merupakan masa setelah persalinan terhitung setelah plasenta keluar. Pada masa nifas terjadi involusi uteri yang dapat berlangsung dengan baik jika di dukung dengan mobilisasi dini. Mobilisasi dini dapat membantu memulihkan keadaan ibu pasca melahirkan dan dapat membant mencegah terjadinya masalah pada masa nifas seperti subinvolusi uteri, infeksi serta perdarahan pada ibu pasca partum. Agar tidak terjadinya masalah tersebut, maka penting bagi tenaga kesehatan untuk memberikan informasi mengenai pentingnya mobilisasi dini untuk mengatasi defisit pengetahuan pada ibu post partum. Penelitian ini bertujuan untuk mengetahui gambaran asuhan keperawatan ibu post partum primipara dengan defisit pengetahuan tentang mobilisasi dini di RSUD Wangaya Tahun 2019. Penelitian ini menggunakan jenis penelitian deskriptif denga pendekatan studi kasus dan teknik pengumpulan data observasi dan dokumentasi. Jumlah subyek yaitu 2 dokumen. Hasil pengakajian meliputi identitas, riwayat obstetri, riwayat penyakit, pola keutuhan sehari-hari, pemeriksaan fisik, diagnose medis, dan pengobatan. Diagnosa medis dirumuskan berdasarkan diagnosa kebidanan. Intervensi di dokumentasikan berdasarkan rencana kebidanan pada post partum pada dokumen Ny.ID dan Ny.J. Implementasi ditulis menggunakan format implementasi keperawatan, impelementasi yang dilakukan yaitu pemberian KIE tentang mobilisasi dini. Evaluasi keperawatan pada dokumen Ny.ID dan Ny.J berbeda di penulisan subjektif, objektif, analisis dan planning karena menggunakan asuhan kebidanan. The postpartum period is the period after labor after the placenta has come out. During the puerperium, uterine involution occurs which can take place well if supported by early mobilization. Early mobilization can help restore the state of postpartum mothers and can help prevent the occurrence of postpartum problems such as uterine subinvolution, infection and bleeding in postpartum mothers. In order not to occur the problem, it is important for health workers to provide information about the importance of early mobilization to overcome the knowledge deficit in post partum mothers. This study aims to describe the nursing care of post-partum primiparous mothers with a deficit of knowledge about early mobilization in Wangaya Hospital in 2019. This study uses a descriptive type of study with a case study approach and a collection of observation and documentation data techniques. The number of subjects is 2 documents. The results of the assessment included identity, obstetric history, disease history, daily integrity patterns, physical examination, medical diagnosis, and treatment. Medical diagnoses are formulated based on obstetric diagnosis. Interventions were documented based on midwifery plans at post partum in Ny. ID and Ny. J documents. The implementation is written using the format of the implementation of nursing, the implementation of which is done is the provision of IEC about early mobilization. Nursing evaluations in Ny. ID and Ny. J documents differ in subjective, objective, analysis and planning writing because they use midwifery care.
