Abstract

MIDWIFERY CARE OF MRS. “SH” 25 YEARS OLD PRIMIGRAVIDA FROM 16 WEEK 3 DAYS PREGNANCY TO 42 DAYS CHILDBIRTH ABSTRACT Continuity of Care (CoC) in midwifery is a model of care provided continuously from the pregnancy period, childbirth, postpartum, newborn, to the selection of contraceptive methods. Each of these stages has the potential for complications that can increase the mortality rate of mothers and infants. The purpose of writing this report is to determine the results of the care provided to Mother "SH" from the second trimester of pregnancy to the postpartum and neonatal periods according to standards. This report uses a case study with data collection techniques thru interviews, examinations, observations, and documentation. Care was provided at UPTD Puskesmas Penebel I from September 2025 to April 2026. During the pregnancy, care was provided according to standards and complemented with prenatal yoga, effleurage massage, and warm compresses. The delivery process occurred spontaneously and according to standards, with complementary care including effleurage massage and counterpressure. The postpartum and neonatal periods were conducted according to standards with the provision of complementary oxytocin massage during the postpartum period and baby massage, which contributed to improving the well-being of both mother and baby. In conclusion, the COC approach is effective in improving the quality of midwifery services and reducing the risk of obstetric complications. It is hoped that this care model can be widely implemented to reduce maternal and infant mortality rates. Keywords: Continuity of Care, midwifery care, pregnancy, labor, postpartum ASUHAN KEBIDANAN PADA IBU “SH” USIA 25 TAHUN PRIMIGRAVIDA DARI USIA KEHAMILAN 16 MINGGU 3 HARI SAMPAI 42 HARI MASA NIFAS ABSTRAK Pelayanan kebidanan berkelanjutan atau Continuity of Care (CoC) merupakan model asuhan yang diberikan secara berkesinambungan mulai dari masa kehamilan, persalinan, nifas, bayi baru lahir, hingga pemilihan alat kontasepsi. Setiap tahapan tersebut memiliki potensi terjadinya komplikasi yang dapat meningkatkan angka kematian ibu maupun bayi. Tujuan penulisan laporan ini yaitu untuk mengetahui hasil penerapan asuhan yang diberikan pada Ibu “SH” dari kehamilan trimester II sampai masa nifas dan neonatus sesuai standar. Laporan ini menggunakan studi kasus dengan teknik pengumpulan data melalui wawancara, pemeriksaan, observasi, dan dokumentasi. Asuhan diberikan di UPTD Puskesmas Penebel I dari bulan September 2025 sampai April 2026. Selama masa kehamilan dilakukan asuhan sesuai standar dan asuhan komplementer berupa prenatal yoga, effluerage massage, dan kompres hangat. Proses persalinan berlangsung secara spontan dan sesuai standar serta diberikan asuhan komplementer berupa effluerage massage dan counterpresurre. Masa nifas dan neonatus dilakukan sesuai standar dengan pemberian komplementer pijat oksitosin selama masa nifas dan pijat bayi yang berkontribusi dalam meningkatkan kesejahteraan ibu dan bayi. Kesimpulannya, pendekatan COC efektif dalam meningkatkan mutu pelayanan kebidanan dan menurunkan risiko komplikasi obstetrik. Diharapkan model perawatan ini dapat diterapkan secara luas untuk menurunkan angka kematian ibu dan bayi. Kata Kunci: Continuity of Care, Asuhan Kebidanan, Kehamilan, Persalinan, Nifas.
