Abstract

ASUHAN KEPERAWATAN NYERI AKUT DENGAN TERAPI KOMPRES DINGIN DAN AROMATERAPI BITTER ORANGE PADA PASIEN POST OPERATIF ORIF DI RS DHARMA YADNYA Ni Made Dwi Satriani Program Studi Profesi Ners Poltekkes Kemenkes Denpasar Email: [email protected] ABSTRAK Fraktur merupakan terputusnya kontinuitas tulang akibat trauma, tekanan, atau kondisi patologis tertentu yang menyebabkan kerusakan jaringan tulang sebagian maupun seluruhnya. Prevalensi fraktur di Indonesia mencapai 5,5% dan di Bali sebesar 7,5%. Masalah keperawatan yang paling sering muncul pada pasien post operatif ORIF (Open Reduction Internal Fixation) adalah nyeri akut akibat agen pencedera fisik (prosedur operasi). Salah satu intervensi non-farmakologis yang dapat diberikan adalah kombinasi terapi kompres dingin dan aromaterapi bitter orange.Tujuan penulisan ini adalah menggambarkan asuhan keperawatan nyeri akut dengan penerapan terapi kompres dingin dan aromaterapi bitter orange pada pasien post operatif ORIF di RS Dharma Yadnya. Metode yang digunakan adalah metode deskriptif dengan pendekatan studi kasus pada satu pasien post operatif ORIF hari pertama dengan diagnosa medis Close fracture right femur middle third di RS Dharma Yadnya yang dilaksanakan pada bulan April 2026. Intervensi dilakukan sebanyak 3 kali pertemuan dengan durasi masing-masing selama 15 menit. Hasil pengkajian awal menunjukkan pasien mengalami nyeri akut dengan skala 5. Setelah diberikan asuhan keperawatan berupa manajemen nyeri, pemberian analgetik, serta kombinasi terapi kompres dingin dan aromaterapi bitter orange, evaluasi akhir menunjukkan nyeri akut menurun. Hal ini ditandai dengan keluhan nyeri berkurang, meringis menurun dari skala 5 menjadi skala 2, gelisah menurun, kesulitan tidur menurun, nafsu makan membaik, dan sikap protektif menurun. Hasil menunjukkan kombinasi terapi kompres dingin dan aromaterapi bitter orange efektif membantu mengatasi masalah keperawatan nyeri akut pada pasien post operatif ORIF. Kata Kunci: Fraktur, Nyeri Akut, Kompres Dingin, Aromaterapi Bitter orange   NURSING CARE FOR ACUTE PAIN UTILIZING COLD COMPRESS THERAPY AND BITTER ORANGE AROMATHERAPY IN POST-OPERATIVE ORIF PATIENTS AT DHARMA YADNYA HOSPITAL By: Ni Made Dwi Satriani Nursing Profession Program, Poltekkes Kemenkes Denpasar Email: [email protected] ABSTRACT A fracture is a disruption in bone continuity caused by trauma, stress, or certain pathological conditions, resulting in partial or complete bone tissue damage. The prevalence of fractures reaches 5.5% in Indonesia and 7.5% in Bali. The most common nursing diagnosis emerging in post-operative ORIF (Open Reduction Internal Fixation) patients is acute pain related to a physical injury agent (surgical procedure). One of the non-pharmacological interventions that can be administered is a combination of cold compress therapy and bitter orange aromatherapy. The purpose of this study is to describe nursing care for acute pain utilizing the application of cold compress therapy and bitter orange aromatherapy in post-operative ORIF patients on first day at Dharma Yadnya Hospital. The method used is a descriptive method with a case study approach on a patient on the first postoperative day of ORIF with a medical diagnosis of a closed fracture of the right middle-third femur at Dharma Yadnya Hospital, conducted in April 2026. The intervention was performed over 3 sessions for 15 minutes each. The initial assessment indicated that the patient experienced acute pain with a scale of 5. After receiving nursing care including pain management, analgesic administration, and a combination of cold compress therapy and bitter orange aromatherapy, the final evaluation showed a decrease in acute pain. This was characterized by reduced pain complaints, a decrease in grimacing from a scale of 5 to 2, reduced restlessness, decreased difficulty sleeping, improved appetite, and decreased protective behavior. The results indicated that the combination of cold compress therapy and bitter orange aromatherapy is effective in addressing the nursing problem of acute pain in post-operative ORIF patients.
