Abstract

GAMBARAN ASUHAN KEPERAWATAN PADA PASIEN GAGAL JANTUNG KONGESTIF DENGAN PENURUNAN CURAH JANTUNG DI RUANG OLEG RSUD MANGUSADA BADUNG TAHUN 2019 ABSTRAK Gagal jantung kongestif adalah ketidakmampuan jantung memompa darah secara adekuat untuk kebutuhan jaringan tubuh sedangkan preload meningkat mengakibatkan darah berlebih masuk ke ventrikel menyebabkan curah jantung menurun. Tanda penurunan curah jantung adalah perubahan irama jantung, ditandai dengan palpitasi. Penelitian ini bertujuan untuk mengetahui gambaran asuhan keperawatan pada pasien gagal jantung kongestif dengan penurunan curah jantung yang meliputi pengkajian sampai evaluasi di ruang Oleg RSUD Mangusada Badung. Penelitian yang digunakan adalah deskirptif dengan rancangan studi kasus. Penelitian dilakukan dengan metode pengumpulan data dari studi dokumentasi yang diamati pada 2 rekam medik pasien gagal jantung kongestif dengan penurunan curah jantung menggunakan lembar observasi berupa check list. Hasil penelitian pada pengkajian tidak ditemukan perbedaan tanda dan gejala dengan teori, diagnosa keperawatan yang ditegakkan penurunan curah jantung, perencanaan sebagian besar berbeda karena perbedaan acuan yang digunakan, implementasi juga ada tindakan tidak dicantumkan pada dokumen pasien, evaluasi menggunakan SOAP dengan perbedaan pada planning. Penelitian menunjukkan terdapat kesamaaan pendokumentasian asuhan keperawatan tetapi ada beberapa perbedaan data dengan teori yang dijadikan acuan oleh peneliti. Hal ini menunjukkan bahwa pendokumentasian asuhan keperawatan dipengaruhi oleh teori yang ditetapkan sebagai standar oleh pihak perawat RSUD Mangusada Badung dan peneliti. Kata kunci : Asuhan Keperawatan, Penurunan Curah Jantung, Gagal Jantung Kongestif DESCRIPTION OF NURSING CARE OF CONGESTIVE HEART FAILURE PATIENT WITH REDUCED CARDIAC OUTPUT IN OLEG WARD MANGUSADA BADUNG HOSPITAL 2019 ABSTRACT Congestive heart failure is the inability of the heart to pump blood adequately for the needs of the body's tissues while increasing preload results in excess blood entering the ventricles causing decreased cardiac output. A sign of a decrease in cardiac output is a change in heart rhythm, marked by palpitations. This study aims to describe the nursing care in patients with congestive heart failure with a decrease in cardiac output which includes assessment until evaluation in the Oleg room at Mangusada Badung Hospital. The research used was descriptive with a case study design. The study was conducted with a method of collecting data from a documentation study which was observed in 2 medical records of patients with congestive heart failure with a decrease in cardiac output using an observation sheet in the form of a check list. The results of the study did not find differences in signs and symptoms with the theory, nursing diagnoses enforced decrease in cardiac output, planning was largely different because of differences in the references used, implementation also had no action included in patient documents, evaluation using SOAP with differences in planning. Research shows that there are similarities in documenting nursing care, but there are several differences in data with the theory used as a reference by researchers. This shows that the documentation of nursing care is influenced by the theory set as standard by the nurses of Mangusada Badung Hospital and researchers.
