Abstract

DESCRIPTION OF NURSING CARE IN PATIENTS DIABETES MELITUS TYPE 2 + DIABETIC FOOT WITH INSTABILITY OF BLOOD GLUCOSE LEVELS ABSTRACT Diabetes Melitus is a metabolic disease characterized by an increase in blood sugar due to abnormal insulin secretion, insulin action or both. 95% of DM found is DM type 2. Complications that often occur in DM type 2 are diabetic foot. Patients with DM type 2 + diabetic foot nursing problems that are often encountered are instability of blood glucose levels. This study aims to describe the nursing care in patients with diabetes melitus type 2 + diabetic foot with instability of blood glucose levels. The research method used was descriptive research with a case study approach in two DM type 2 + diabetic foot patients. The study was conducted in April 2019. The data collection tool used is the documentation observation guide in the form of a check list. The results of this case study, the study of data, diagnoses, interventions and implementation there are few differences with theoretical references. The evaluation uses SOAP but there are differences in thesection assement. This happened because of differences in the theory used in the Cendrawasih Ward of Wangaya Hospital. From this research, it is hoped that it can be used as a guideline for educational institutions, Wangaya Hospital, and further researchers. Keywords: nursing care, DM type 2 + diabetic foot, instability of blood glucose levels GAMBARAN ASUHAN KEPERAWATAN PADA PASIEN DIABETES MELITUS TIPE 2 + DIABETIC FOOT DENGAN KETIDAKSTABILAN KADAR GLUKOSA DARAH ABSTRAK Diabetes Melitus adalah penyakit metabolik ditandai dengan terjadinya peningkatan gula darah akibat kelainan sekresi insulin, kerja insulin atau keduanya. 95% DM yang dijumpai DM tipe 2. Komplikasi yang sering terjadi pada DM tipe 2 adalah diabetic foot. Pasien DM tipe 2 + diabetic foot masalah keperawatan yang muncul adalah ketidakstabilan kadar glukosa darah. Penelitian ini bertujuan untuk mengetahui gambaran asuhan keperawatan pada pasien diabetes melitus tipe 2 + diabetic foot dengan ketidakstabilan kadar glukosa darah. Metode penelitian yang digunakan yaitu penelitian deskriptif dengan pendekatan studi kasus pada dua pasien DM tipe 2 + diabetic foot. Penelitian dilakukan pada bulan April 2019. Alat pengumpulan data yang digunakan adalah pedoman observasi dokumentasi yang berupa check list. Hasil penelitian studi kasus ini, pengkajian data, diagnosa, intervensi dan implementasi terdapat sedikit perbedaan dengan acuan teori. Pada evaluasi menggunakan SOAP namun ada perbedaan pada bagian assement. Hal ini terjadi karena perbedaan teori yang digunakan di Ruang Cendrawasih RSUD Wangaya. Penelitian ini diharapkan dapat dijadikan pedoman baik bagi institusi pendidikan, RSUD Wangaya, serta peneliti selanjutnya.
