Abstract

MIDWIFERY CARE FOR “KW,” A 29-YEAR-OLD MULTIPAROUS WOMAN FROM 20 WEEKS AND 2 DAYS OF GESTATION THROUGH 42 DAYS OF THE POSTNATAL PERIOD ABSTRACT Pregnancy is a physiological process that requires continuous monitoring to maintain the health of the mother and fetus and to detect complications early. Therefore, the implementation of midwifery care through Continuity of Care (COC) is carried out from pregnancy through the postpartum period to improve the quality of care and the health status of the mother and baby. This report aims to determine the outcomes of midwifery care provided from 20 weeks and 2 days of gestation through 42 days of the postpartum period, along with care for the infant, all delivered in accordance with established standards. This study employed a case study design, utilizing data collection techniques including interviews, examinations, observations, and documentation. Care was provided from October 2025 to April 2026. The care results indicated that the pregnancy progressed physiologically in accordance with the 12T care standards; labor proceeded normally with Stage I lasting 2 hours, Stage II 25 minutes, Stage III 5 minutes, and Stage IV monitoring within normal limits. The infant cried immediately upon birth, exhibited active muscle tone, had a rosy complexion, and weighed 3,100 grams at birth. The process of uterine involution, lochia discharge, and lactation during the postpartum period proceeded normally. Complementary care was also effectively provided according to the mother’s needs. The entire midwifery care process, conducted under the continuity of care (COC) model in accordance with standards, yielded optimal results: a physiological pregnancy, normal delivery, a healthy postpartum period, and a baby developing without complications. It is recommended that healthcare providers continue to provide comprehensive and continuous care. Keywords: Pregnancy, Childbirth, Newborn, Postpartum ASUHAN KEBIDANAN PADA IBU “KW” UMUR 29 TAHUN MULTIGRAVIDA DARI UMUR KEHAMILAN 20 MINGGU 2 HARI SAMPAI 42 HARI MASA NIFAS ABSTRAK Kehamilan merupakan proses fisiologis yang memerlukan pemantauan secara berkesinambungan untuk menjaga kesehatan ibu dan janin serta mendeteksi dini komplikasi Oleh karena itu, penerapan asuhan kebidanan secara Continuity of Care (COC) dilakukan sejak masa kehamilan hingga masa nifas untuk meningkatkan kualitas pelayanan dan derajat kesehatan ibu dan bayi. Laporan ini bertujuan mengetahui hasil penerapan asuhan kebidanan sejak usia kehamilan 20 minggu 2 hari sampai 42 hari masa nifas beserta bayinya yang diberikan sesuai standar. Studi ini menggunakan studi kasus dengan teknik pengumpulan data melalui wawancara, pemeriksaan, observasi serta dokumentasi. Asuhan diberikan dari Oktober 2025 sampai April 2026. Hasil asuhan menunjukkan bahwa kehamilan berlangsung fisiologis sesuai standar asuhan 12T, persalinan berjalan normal dengan Kala I 2 jam, kala II 25 menit, kala III 5 menit dan pemantauan kala IV dalam batas normal. Bayi lahir segera menangis, tonus otot aktif, kulit kemerahan dan berat lahir 3.100 gram. Proses involusi uterus, pengeluaran lochea dan laktasi pada masa nifas berlangsung normal. Asuhan komplementer juga diterapkan dengan baik sesuai kebutuhan ibu. Seluruh proses asuhan kebidanan secara continuity of care (COC) yang dilakukan sesuai standar dengan hasil optimal, kehamilan fisiologis, persalinan normal, nifas baik dan bayi berkembang tanpkomplikasi.
