Abstract

MIDWIFERY CARE OF MRS. “MA” 21 YEARS OLD MULTIGRAVIDA FROM 38 WEEKS 5 DAYS GESTATION TO 42 DAYS POSTPARTUM The case report was conducted in the working area of the Regional Technical Implementation Unit of Mengwi 1 Community Health Center ABSTRACT The high maternal and infant mortality rate remains a health problem that requires comprehensive and continuous midwifery care through a continuity of care (CoC) approach. This study aimed to describe the implementation of comprehensive midwifery care for Mrs. “MA,” a 21-year-old multigravida (G2P0A1), from 38 weeks and 5 days of pregnancy until 42 days postpartum. The care was conducted at the working area of UPTD Puskesmas Mengwi 1 in 2026 using a descriptive case study method through interviews, observation, physical examination, and documentation. The results showed that the pregnancy progressed physiologically. Pregnancy care was provided according to the 12T ANC standard accompanied by health education and birthing ball therapy. Labor occurred spontaneously, normally, and without complications with partograph monitoring and active management of the third stage of labor. The baby was born healthy with a birth weight of 3290 grams, cried strongly, moved actively, had reddish skin color, and early initiation of breastfeeding (IMD) was performed. Postpartum and newborn care included monitoring the condition of the mother and baby, breastfeeding education, cord care, administration of vitamin K, eye ointment, and hepatitis B immunization. The postpartum and neonatal periods progressed physiologically without complications. The conclusion of this study is that the implementation of continuity of care midwifery services according to standards can support maternal and infant health and assist in the early detection of complications. ASUHAN KEBIDANAN PADA NY. “MA” UMUR 21 TAHUN MULTIGRAVIDA DARI UMUR KEHAMILAN 38 MINGGU 5 HARI SAMPAI 42 HARI MASA NIFAS Laporan Kasus Dilaksanakan di Wilayah Kerja Unit Pelaksanaan Teknis Daerah Puskesmas Mengwi 1 ABSTRAK Tingginya angka kematian ibu dan bayi masih menjadi masalah kesehatan yang memerlukan pelayanan kebidanan secara komprehensif dan berkesinambungan melalui pendekatan continuity of care (CoC). Penelitian ini bertujuan menggambarkan penerapan asuhan kebidanan komprehensif pada Ny. “MA” umur 21 tahun multigravida (G2P0A1) dari usia kehamilan 38 minggu 5 hari sampai 42 hari masa nifas. Asuhan dilaksanakan di wilayah kerja UPTD Puskesmas Mengwi 1 tahun 2026 menggunakan metode studi kasus deskriptif melalui wawancara, observasi, pemeriksaan fisik, dan dokumentasi. Hasil asuhan menunjukkan kehamilan berlangsung fisiologis. Asuhan kehamilan diberikan sesuai standar ANC 12T disertai edukasi kesehatan dan komplementer birthing ball. Persalinan berlangsung spontan, normal, dan tanpa komplikasi dengan pemantauan partograf serta manajemen aktif kala III. Bayi lahir sehat dengan berat badan 3290 gram, menangis kuat, gerak aktif, kulit kemerahan, dan dilakukan IMD. Asuhan nifas dan bayi baru lahir diberikan melalui pemantauan kondisi ibu dan bayi, edukasi menyusui, perawatan tali pusat, pemberian vitamin K, salep mata, dan imunisasi hepatitis B. Masa nifas dan neonatus berlangsung fisiologis tanpa komplikasi. Kesimpulan penelitian ini adalah penerapan asuhan kebidanan continuity of care sesuai standar dapat mendukung kesehatan ibu dan bayi serta membantu deteksi dini komplikasi.
