Abstract

Congestive heart failure is when heart failed pump adequet blood for the body's needs while the preload is increased due to excess blood entering ventricles as a result of reduced cardiac output. Sign of reduced cardiac output is the change of cardiac contractility, characterized by reduced ejection fraction. This study aims to determine nursing care decription of congestive heart failure with reduced cardiac output including assessment to evaluation in Oleg Ward Mangusada Badung Hospital. Research type in this study is descriptive with case study design. Data collected using documentary study in the medical record on two congestive heart failure patients with reduced cardiac output. The conclusion of this study are 1)assessment have only two difference data which are pulse rate and blood pressure and there was no data of ejection fraction as in the theory, 2)diagnosis, intervention and implementation patients had the same data in both patients, there was also little difference in theoretical references, 3)evaluation similar to theory using SOAP format but there was one of the outcome criteria that didn’t record in the nursing evaluation, 4)After being given a nursing care for 3x24 hours, the problem was not resolved and nursing interventions were still continuing. Based on the conclusion of the study, here are several recommendations to be considered, 1)Healthcare provider and patients need to have good cooperation in order to provide optimal nursing care for patients. 2)The educational institution need to give special time for research in order to obtain optimum result.
