Abstract

ANALYSIS OF DIFFERENCES IN LEUKOCYTE, ABSOLUTE NEUTROPHIL COUNT AND HEMOGLOBIN VALUES BASED ON CHEMOTHERAPY CYCLES IN BREAST CANCER PATIENTS AT BALI MANDARA HOSPITAL ABSTRACT Breast Breast cancer is the most common cancer among women, and chemotherapy remains one of the primary treatment modalities. However, chemotherapy may induce myelosuppression, leading to decreased hematological parameters, particularly leukocyte count, Absolute Neutrophil Count (ANC), and hemoglobin. This study aimed to identify leukocyte, ANC, and hemoglobin levels before and after chemotherapy and to analyze their differences across chemotherapy cycles in breast cancer patients at Bali Mandara Regional General Hospital. This analytical observational study employed a retrospective design using 119 medical records selected through purposive sampling. Data were analyzed using the Wilcoxon Signed Rank Test, Kruskal-Wallis test, and post hoc analysis. The results demonstrated significant differences in leukocyte, ANC, and hemoglobin levels before and after chemotherapy (p<0.05). Changes in leukocyte count and ANC differed significantly across chemotherapy cycles (p<0.05), whereas changes in hemoglobin did not (p>0.05). In conclusion, chemotherapy is associated with changes in hematological parameters in breast cancer patients, with leukocyte count and ANC being more sensitive indicators than hemoglobin for monitoring chemotherapy-induced myelosuppression across treatment cycles. Namun, kemoterapi dapat menyebabkan mielosupresi yang berdampak pada penurunan parameter hematologi, terutama leukosit, neutrofil absolut (Absolute Neutrophil Count/ANC), dan hemoglobin. Penelitian ini bertujuan mengidentifikasi nilai leukosit, ANC, dan hemoglobin sebelum dan sesudah kemoterapi serta menganalisis perbedaannya berdasarkan siklus kemoterapi pada pasien kanker payudara di RSUD Bali Mandara. Penelitian ini menggunakan desain observasional analitik dengan pendekatan retrospektif terhadap 119 data rekam medis pasien yang dipilih secara purposive sampling. Analisis data menggunakan uji Wilcoxon Signed Rank Test, Kruskal-Wallis, dan uji lanjut (post hoc). Hasil penelitian menunjukkan terdapat perbedaan bermakna nilai leukosit, ANC, dan hemoglobin sebelum dan sesudah kemoterapi (p<0,05). Perubahan leukosit dan ANC berbeda bermakna berdasarkan siklus kemoterapi (p<0,05), sedangkan perubahan hemoglobin tidak menunjukkan perbedaan bermakna (p>0,05). Disimpulkan bahwa kemoterapi berhubungan dengan perubahan parameter hematologi pada pasien kanker payudara, dengan leukosit dan ANC merupakan parameter yang lebih sensitif dibandingkan hemoglobin dalam menggambarkan efek mielosupresif berdasarkan siklus kemoterapi.
