Abstract
NURSING CARE FOR DELUSIONS USING GUIDED IMAGERY THERAPY IN SCHIZOPHRENIC PATIENTS AT THE KUNTI WARD OF MANAH SHANTI MAHOTTAMA HOSPITAL IN 2026 Ni Wayan Ayu Puspita Dewi Nursing Professional Program, Denpasar Public Health Polytechnic, Ministry of Health Email: [email protected] ABSTRACT Delusional disorders in patients with schizophrenia are a mental health nursing issue characterized by false, persistent beliefs that do not correspond to reality, thereby affecting the patients’ behavior, social functioning, and risk of violence. Suboptimal management can increase the risk of relapse and prolong the duration of treatment. This scientific study aims to examine nursing care for delusional disorders through the application of delusion management and reality orientation combined with guided imagery therapy in patients with schizophrenia in the Kunti Ward at Manah Shanti Mahottama Hospital. The assessment results for Mrs. S, a 45-year-old woman with hebephrenic schizophrenia, revealed that the patient believed herself to be the reincarnation of the god Gatot Kaca. The established nursing diagnosis was delusions related to social isolation as the primary issue. The intervention was implemented over 10 sessions, encompassing the development of a trusting relationship, delusion management and reality orientation, collaborative administration of antipsychotic medication, and four sessions of guided imagery therapy. Evaluation results showed a significant decrease in the frequency and intensity of verbalized delusions, improved concentration, a calmer and more cooperative patient, and the ability to independently control delusional thoughts using guided imagery therapy starting from the second session. The combination of delusion management and reality orientation as primary interventions, reinforced by guided imagery therapy as a supportive intervention, was effective in me reduce symptoms of delusional disorder and improve schizophrenia patients’ ability to control their internal cognitive distortions. ASUHAN KEPERAWATAN WAHAM DENGAN TERAPI GUIDED IMAGERY PADA PASIEN SKIZOFRENIA DI RUANG KUNTI RUMAH SAKIT MANAH SHANTI MAHOTTAMA TAHUN 2026 Ni Wayan Ayu Puspita Dewi Program Studi Profesi Ners Poltekkes Kemenkes Denpasar Email: [email protected] ABSTRAK Gangguan waham pada pasien skizofrenia merupakan masalah keperawatan jiwa yang ditandai dengan keyakinan yang salah, menetap, dan tidak sesuai kenyataan, sehingga berdampak pada perilaku, fungsi sosial, dan risiko kekerasan pasien. Penanganan yang kurang optimal dapat meningkatkan risiko kekambuhan dan memperpanjang masa perawatan. Karya ilmiah ini bertujuan untuk mengetahui asuhan keperawatan gangguan waham melalui penerapan manajemen waham dan orientasi realita yang dikombinasikan dengan terapi guided imagery pada pasien skizofrenia di Ruang Kunti Rumah Sakit Manah Shanti Mahottama. Hasil pengkajian pada Ny. S, perempuan usia 45 tahun dengan skizofrenia hebefrenik, menunjukkan pasien meyakini dirinya titisan Dewa Gatot Kaca. Diagnosis keperawatan yang ditegakkan adalah waham berhubungan dengan isolasi sosial sebagai masalah utama. Implementasi dilaksanakan dalam 10 kali pertemuan mencakup pembinaan hubungan saling percaya, manajemen waham dan orientasi realita, kolaborasi pemberian obat antipsikotik, serta terapi guided imagery sebanyak 4 sesi. Hasil evaluasi menunjukkan penurunan frekuensi dan intensitas verbalisasi waham secara bermakna, peningkatan konsentrasi, pasien lebih tenang dan kooperatif, serta mampu mengontrol pikiran waham secara mandiri menggunakan terapi guided imagery sejak sesi kedua. Kombinasi intervensi manajemen waham dan orientasi realita sebagai intervensi utama, yang diperkuat dengan terapi guided imagery sebagai intervensi pendukung, efektif dalam menurunkan gejala gangguan waham dan meningkatkan kemampuan kontrol diri pasien skizofrenia terhadap distorsi kognitif internal.
