Abstract

IMPLEMENTATION OF CHEST PHYSIOTHERAPY TO IMPROVE AIRWAY CLEARANCE IN PRESCHOOL CHILDREN (3-6 YEARS) WITH PNEUMONIA AT KLUNGKUNG HOSPITAL ABSTRACT Pneumonia is an inflammatory condition of the lung parenchyma in which the acinuses are filled with inflammatory fluid, with or without inflammatory cells entering the alveoli walls and interstitial cavity. This condition often produces excessive secretions in the lungs. The secretion or phlegm will accumulate and become thick, making it difficult to expel which can trigger a cough response, making the patient experience ineffective airway clearance. This condition requires appropriate treatment to remove phlegm or sputum that accumulates in the respiratory tract, one of which is non-pharmacological techniques, namely chest physiotherapy. The purpose of this study was to determine how the implementation of chest physiotherapy to improve airway clearance in preschool children (3-6 years) with pneumonia. This research used a descriptive case study type with an in- depth case study form on one subject in Bakas Room, Klungkung Hospital. The results of this study indicate that there is an improvement in airway clearance in children as evidenced by not appearing restless and starting to feel comfortable when spoken to, appearing to be able to cough effectively, sputum production decreases, wheezing sounds decrease, breathing frequency begins to improve, RR. 25x/min. The conclusion of this study is that chest physiotherapy techniques can improve airway clearance in children with pneumonia. Keywords: Pneumonia, Ineffective Airway Clearance, Chest Physiotherapy IMPLEMENTASI FISIOTERAPI DADA UNTUK MENINGKATKAN BERSIHAN JALAN NAPAS PADA ANAK USIA PRASEKOLAH (3-6 TAHUN) DENGAN PNEUMONIA DI RSUD KLUNGKUNG ABSTRAK Pneumonia adalah kondisi peradangan parenkim paru di mana asinus dipenuhi dengan cairan radang, dengan atau tanpa sel radang masuk ke dalam dinding alveoli dan rongga interstisium. Kondisi ini seringkali memproduksi sekret berlebihan pada paru-paru. Sekret atau dahak akan menumpuk dan menjadi kental, sehingga sulit untuk dikeluarkan yang dapat memicu respon batuk sehingga membuat pasien mengalami bersihan jalan napas tidak efektif. Kondisi tersebut memerlukan penanganan yang sesuai untuk mengeluarkan dahak atau sputum yang menumpuk di saluran pernapasan, salah satunya teknik nonfarmakologis yaitu fisioterapi dada. Tujuan penelitian ini yaitu untuk mengetahui bagaimana implementasi fisioterapi dada untuk meningkatkan bersihan jalan napas pada anak prasekolah (3-6 tahun) dengan pneumonia. Penelitian ini menggunakan jenis studi kasus deskriptif dengan bentuk studi kasus mendalam pada satu orang subjek di Ruang Bakas RSUD Klungkung. Hasil penelitian ini menunjukkan bahwa terdapat peningkatan bersihan jalan napas pada anak dibuktikan dengan tidak tampak gelisah dan mulai merasa nyaman saat diajak berbicara, tampak mampu melakukan batuk efektif, produksi sputum menurun, suara wheezing menurun, frekuensi napas mulai membaik, RR: 25x/menit. Kesimpulan dari penelitian ini didapatkan teknik fisioterapi dada mampu meningkatkan bersihan jalan napas pada anak dengan pneumonia.
