Abstract

ABSTRACT Tuberculosis (TBC) remains a major public health challenge in Indonesia. Ile Ape Subdistrict, Lembata Regency recorded an 81% increase in TBC cases over three years (2023–2025), spreading across 16 of 17 villages, despite a healthy-house coverage rate of 92–98%. This paradox suggests that general housing assessments fail to capture indoor environmental risk factors directly influencing airborne TBC transmission. This study aimed to analyze the relationship between physical housing quality and TBC incidence in the working area of Puskesmas Waipukang, Ile Ape Subdistrict. A retrospective case-control study was conducted involving 30 cases and 30 controls. Data were collected through direct measurement of lighting (lux meter), ventilation (tape measure), and indoor humidity (hygrometer) in respondents' bedrooms, analyzed using Chi-Square, Odds Ratio (OR), and Contingency Coefficient (CC) in SPSS, referencing Permenkes No. 2 of 2023. Results showed that 45.0% of houses did not meet physical quality standards, with lighting as the most disparate component. Bivariate analysis revealed a highly significant association between physical housing quality and TB incidence (p<0.001; OR=188.500; 95% CI: 19.782–1,796.212). Lighting and ventilation were individually significant (p<0.001), while humidity was not (p=0.706). Improving lighting and ventilation should be prioritized as environmentally targeted TB control interventions. Keywords: tuberculosis, physical housing quality. ABSTRAK Tuberkulosis (TBC) masih menjadi tantangan kesehatan masyarakat yang besar di Indonesia. Kecamatan Ile Ape, Kabupaten Lembata mencatat peningkatan kasus TBC sebesar 81% dalam tiga tahun (2023–2025), tersebar di 16 dari 17 desa, meskipun cakupan rumah sehat mencapai 92–98%. Paradoks ini mengindikasikan bahwa penilaian rumah secara umum tidak mampu menangkap faktor risiko lingkungan dalam ruangan yang secara langsung mempengaruhi penularan TBC melalui udara. Penelitian ini bertujuan menganalisis hubungan antara kualitas fisik rumah dengan kejadian TBC di wilayah kerja Puskesmas Waipukang, Kecamatan Ile Ape. Penelitian analitik observasional retrospektif dengan desain case-control dilakukan terhadap 30 kasus dan 30 kontrol. Data dikumpulkan melalui pengukuran langsung pencahayaan (lux meter), ventilasi (meteran), dan kelembaban dalam ruangan (hygrometer) di kamar tidur responden, dianalisis menggunakan Chi-Square, Odds Ratio (OR), dan Koefisien Kontingensi (KK) pada SPSS, mengacu pada Permenkes No. 2 Tahun 2023. Hasil menunjukkan 45,0% rumah tidak memenuhi standar kualitas fisik, dengan pencahayaan sebagai komponen paling timpang. Analisis bivariat menunjukkan hubungan yang sangat bermakna antara kualitas fisik rumah dengan kejadian TB (p<0,001; OR=188,500; 95% CI: 19,782–1.796,212). Pencahayaan dan ventilasi bermakna secara individual (p<0,001), sedangkan kelembaban tidak bermakna (p=0,706). Peningkatan pencahayaan dan ventilasi perlu diprioritaskan sebagai intervensi pengendalian TB berbasis lingkungan. Kata kunci: tuberkulosis, kualitas fisik rumah.
