Abstract

THE RELATIONSHIP BETWEEN REPEATED BLOOD TRANSFUSIONS AND CROSSMATCH RESULTS IN PATIENTS WITH THALASSEMIA MAJOR AT PROF. DR. I.G.N.G. NGOERAH GENERAL HOSPITAL ABSTRACT Backround: Thalassemia major is a hereditary hemolytic disorder that requires lifelong repeated blood transfusions. Continuous exposure to donor erythrocyte antigens may stimulate alloantibody formation, increasing the risk of incompatible crossmatch results and complicating safe transfusion therapy. Objective: This study aimed to determine the relationship between repeated blood transfusions and crossmatch results among patients with thalassemia major at Prof. Dr. I.G.N.G. Ngoerah General Hospital. Methods: This research used an analytical observational design with a quantitative retrospective approach. A total of 81 patients diagnosed with thalassemia major and receiving routine blood transfusions from 2023 to 2025 were included using total sampling. Secondary data were obtained from medical records and analyzed to assess transfusion frequency and crossmatch compatibility. Result: The relationship was analyzed using Spearman's rank correlation test. The results showed a significant relationship between repeated blood transfusions and compatible crossmatch results, with a p-value of 0.000 and a correlation coefficient of 0.851. A significant relationship was also found between repeated blood transfusions and incompatible crossmatch results, with a p-value of 0.000 and a correlation coefficient of 0.723. These findings indicate a strong positive correlation, suggesting that increased transfusion frequency is associated with a higher likelihood of incompatible crossmatch results. Therefore, regular antibody screening and careful transfusion management are essential to improve patient safety. Conclusion: There is an association between repeated blood transfusions and crossmatch results in patients with thalassemia major at Prof. Dr. I.G.N.G. Ngoerah General Hospital. Keywords: Crossmatch, thalassemia major, transfusion. HUBUNGAN TRANSFUSI DARAH BERULANG DENGAN HASIL CROSSMATCH PADA PASIEN THALASEMIA MAYOR DI RSUP PROF. DR. I G. N. G. NGOERAH ABSTRAK Latar Belakang: Thalasemia mayor merupakan kelainan hemolitik herediter yang membutuhkan transfusi darah berulang, namun paparan antigen eritrosit donor secara terus-menerus dapat memicu alloimunisasi sehingga berisiko menyebabkan hasil crossmatch inkompatibel dan menghambat pelayanan transfusi darah yang aman. Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan antara transfusi darah berulang dengan hasil crossmatch pada pasien thalasemia mayor di RSUP Prof. Dr. I.G.N.G. Ngoerah. Metode: Penelitian ini merupakan penelitian observasional analitik dengan pendekatan kuantitatif dan desain retrospektif pada 81 pasien secara total sampling. Data yang dikumpulkan adalah data sekunder yang diperoleh dari rekam medis pasien dengan diagnosis thalasemia mayor dan menjalankan transfusi darah rutin pada periode tahun 2023 hingga tahun 2025. Hasil: Hasil analisis hubungan menggunakan uji statistik Spearman Rank. Hasil penelitian menunjukkan terdapat hubungan yang signifikan antara transfusi darah berulang dengan hasil crossmatch kompatibel (p value = 0,000) dan koefisien korelasi sebesar 0,851. Terdapat hubungan yang signifikan antara transfusi darah berulang dengan hasil crossmatch inkompatibel (p value = 0,000) dan koefisien korelasi sebesar 0,723. Nilai tersebut menunjukkan hubungan positif dengan tingkat hubungan kuat, yang berarti semakin tinggi jumlah transfusi darah berulang, maka semakin tinggi pula kemungkinan ditemukannya hasil crossmatch inkompatibel. Kesimpulan: terdapat hubungan antara transfusi darah berulang dengan hasil crossmatch pada pasien thalasemia mayor di RSUP Prof. Dr. I.G.N.G Ngoerah.
