Abstract
CORRELATION BETWEEN TACROLIMUS CONCENTRATION WITH SERUM CREATININE ON POST KIDNEY TRANSPLANTATION PATIENT AT PROF dr I G N G NGOERAH HOSPITAL ABSTRACT Background: Kidney failure is a condition on which kidney function decrease irreversibly. Kidney graft transplantation often chosen as medication for its better long term effect than dyalisis. Kidney graft rejection often occur on patient and therefore tacrolimus is administrated to prevent it although it has nefrotoxic effect. Serum creatinine test is commonly used to monitor its nefrotoxicity effect on kidney. Aim: This study aimed to analyze the correlation between tacrolimus concentration and serum creatinine on post kidney transplantation patients at Prof Ngoerah hospital Bali. Methods: a quantitative observational analytic with cross-sectional design was applied using secondary data from 46 post kidney transplantation patients who underwent tacrolimus and serum creatinine testing. Data were analyzed with Spearman rank correlation test. Results: there is increasing mean on tacrolimus test from 4,385 to 4,937 after 5 times and end period of tacrolimus consumption. Serum creatinine mean increase from 1,405 to 2,8735 after 5 times and end period of tacrolimus consumption that sugest serum creatinine increasing throughout tacrolimus consumption. No correlation found between tacrolimus and serum creatinine after 5 times (r =-0,049; p = 0,747) and at the end of tacrolimus consumption (r=0,083; p=0,585). Conculsion: The results indicate no correlation between tacrolimus and serum creatinine. However, it is necessary to carry out new studies to understand better the impact of tacrolimus on serum creatinine. Keyword: Tacrolimus, serum creatinine, graft kidney transplantation ANALISIS HUBUNGAN KADAR TACROLIMUS DENGAN SERUM CREATININ PADA PASIEN POST TRANSPLANTASI GINJAL DI RUMAH SAKIT UMUM PUSAT Prof dr I G N G NGOERAH ABSTRAK Latar Belakang: Gagal ginjal adalah kondisi dimana fungsi ginjal menurun secara irreversible. Transplantasi ginjal sering menjadi pengobatan yang lebih dipilih karena efek jangka panjangnya. Reaksi penolakan setelah transplantasi sering terjadi sehingga obat tacrolimus digunakan untuk menekan reaksi penolakan tubuh walaupun mempunyai efek nefrotoksik. Sementara serum creatinine merupakan parameter uji yang umum untuk menilai kerusakan ginjal. Tujuan: Penelitian ini bertujuan untuk menganalisis hubungan antara tacrolimus dengan serum creatinine pada pasien post transplantasi ginjal di RSUP Prof Ngoerah Bali. Metode: penelitian ini adalah penelitian observasional analitik dengan desain potong lintang dengan 46 data rekam medis pasien post transplantasi yang menjalani pemeriksaan tacrolimus dan serum creatinine. Hasil: terdapat peningkatan mean tacrolimus dari 4,385 to 4,937 pada 5 kali setelah konsumsi dan akhir konsumsi tacrolimus. Terdapat peningkatan mean serum creatinine dari 1,405 to 2,8735 pada 5 kali setelah konsumsi dan akhir konsumsi tacrolimus yang mengindikasikan peningkatan serum creatinine selama konsumsi tacrolimus. Tidak ditemukan hubungan yang signifikan antara tacrolimus dan serum creatinine setelah 5 kali konsumsi tacrolimus (r =-0,049; p = 0,747) dan pada akhir konsumsi tacrolimus (r=0,083; p=0,585). Kesimpulan : hasil penelitian menunjukkan tidak terdapat hubungan antara tacrolimus dengan serum creatinine. Namun, perlu dilakukan studi lainnya untuk memahami hubungan antara tacrolimus dengan serum creatinine Kata kunci: Tacrolimus, serum creatinine, transplantasi ginjal
