Abstract

THE ASSOCIATION BETWEEN PACKED RED CELLS TRANSFUSION FREQUENCY AND FERRITIN LEVELS AND TRANSAMINASE ENZYMES IN CHILDREN WITH BETA-THALASSEMIA MAJOR AT PROF. DR. I.G.N.G. NGOERAH GENERAL HOSPITAL DENPASAR ABSTRACT Beta-thalassemia major is a hereditary hemolytic disease that requires regular Packed Red Cells (PRC) transfusions from childhood. Repeated transfusions may lead to iron accumulation in the body, indicated by increased serum ferritin levels, and may potentially cause liver function impairment through elevated transaminase enzymes. This study aimed to analyze the relationship between PRC transfusion frequency and serum ferritin and transaminase enzyme levels in children with beta-thalassemia major at Prof. Dr. I.G.N.G. Ngoerah General Hospital, Denpasar. This was an analytical observational study with a quantitative approach and a cross-sectional design. The study used secondary data from medical records of pediatric patients with beta-thalassemia major who underwent routine transfusions from February to April 2026. The research variables included PRC transfusion frequency, serum ferritin levels, SGOT, and SGPT. The results showed a relationship between PRC transfusion frequency and serum ferritin levels (p=0.020), with a weak positive correlation. No relationship was found between PRC transfusion frequency and SGOT levels (p=0.335). However, there was a relationship between PRC transfusion frequency and SGPT levels (p=0.010), with a positive correlation. In conclusion, increased PRC transfusion frequency is associated with increased serum ferritin and SGPT levels but is not associated with SGOT levels. I.G.N.G NGOERAH DENPASAR ABSTRAK Thalasemia beta mayor merupakan penyakit hemolitik herediter yang memerlukan transfusi Packed Red Cells (PRC) secara rutin sejak usia anak. Transfusi berulang dapat menyebabkan akumulasi zat besi dalam tubuh yang ditandai dengan peningkatan kadar feritin serum dan berpotensi menimbulkan gangguan fungsi hati melalui peningkatan enzim transaminase. Penelitian ini bertujuan untuk menganalisis hubungan antara frekuensi transfusi PRC dengan kadar feritin serum dan enzim transaminase pada anak thalasemia beta mayor di RSUP Prof. Dr. I.G.N.G. Ngoerah Denpasar. Penelitian ini merupakan penelitian observasional analitik dengan pendekatan kuantitatif dan desain potong lintang. Data yang digunakan adalah data sekunder dari rekam medis pasien anak thalasemia beta mayor yang menjalani transfusi rutin pada Februari–April 2026. Variabel penelitian meliputi frekuensi transfusi PRC, kadar feritin serum, SGOT, dan SGPT. Hasil penelitian menunjukkan terdapat hubungan antara frekuensi transfusi PRC dengan kadar feritin serum (p=0,020) dengan kekuatan hubungan lemah dan arah positif. Tidak terdapat hubungan antara frekuensi transfusi PRC dengan kadar SGOT (p=0,335). Namun, terdapat hubungan antara frekuensi transfusi PRC dengan kadar SGPT (p=0,010) dengan arah positif. Kesimpulannya, peningkatan frekuensi transfusi PRC berhubungan dengan peningkatan kadar feritin serum dan SGPT, tetapi tidak berhubungan dengan kadar SGOT.
