Abstract
HUBUNGAN BACTERIAL LOAD MYCOBACTERIUM TUBERCULOSIS PADA PASIEN TUBERKULOSIS PARU DENGAN REAKTIVITAS TUBERCULIN SKIN TEST PADA KONTAK SERUMAH ABSTRAK Latar belakang: Tuberkulosis (TB) masih menjadi masalah kesehatan masyarakat, terutama pada kontak serumah pasien TB paru. Pemeriksaan Tes Cepat Molekuler (TCM) digunakan untuk menentukan bacterial load Mycobacterium tuberculosis pasien TB paru, sedangkan Tuberculin Skin Test (TST) digunakan untuk menilai respons imun terhadap paparan Mycobacterium tuberculosis pada kontak serumah.Tujuan: Penelitian ini bertujuan menganalisis hubungan antara bacterial load Mycobacterium tuberculosis pada pasien TB paru dengan reaktivitas TST pada kontak serumah. Metode: Penelitian ini menggunakan desain observasional analitik dengan pendekatan retrospektif. Sampel penelitian berjumlah 100 kontak serumah dari 32 pasien TB paru yang memenuhi kriteria inklusi. Data bacterial load Mtb dan reaktivitas TST diperoleh dari Sistem Informasi Tuberkulosis (SITB) UPTD Puskesmas I Dinas Kesehatan Kecamatan Denpasar Utara. Analisis dilakukan menggunakan uji chi-square dan dilanjutkan dengan Monte Carlo exact test. Hasil: Sebagian besar responden berusia 18–59 tahun (74%) dan berjenis kelamin laki-laki (53%). Distribusi kontak serumah berdasarkan bacterial load Mtb pada pasien TB paru didominasi kategori low (30%), sedangkan reaktivitas TST kontak serumah didominasi hasil negatif (78%). Analisis menunjukkan hubungan signifikan antara bacterial load Mtb dan reaktivitas TST dengan nilai p = 0,001 (p < 0,05). Kesimpulan: terdapat hubungan signifikan antara bacterial load Mtb berdasarkan hasil TCM pada pasien TB paru dengan reaktivitas TST pada kontak serumah. Kata kunci: Bacterial load, Tuberkulosis. THE CORRELATION BETWEEN MYCOBACTERIUM TUBERCULOSIS BACTERIAL LOAD IN PATIENTS WITH PULMONARY TUBERCULOSIS AND TUBERCULIN SKIN TEST REACTIVITY AMONG HOUSEHOLD CONTACTS ABSTRACT Background: Tuberculosis (TB) remains a major public health problem, particularly among household contacts of patients with pulmonary TB. The Molecular Rapid Test (TCM) is used to determine the bacterial load of Mycobacterium tuberculosis in patients with pulmonary TB, while the Tuberculin Skin Test (TST) is used to assess the immune response to Mycobacterium tuberculosis exposure among household contacts. Objective: This study aimed to analyze the relationship between the Mycobacterium tuberculosis bacterial load in patients with pulmonary TB and TST reactivity among household contacts. Methods: This study employed an analytical observational design with a retrospective approach. The sample consisted of 100 household contacts from 32 patients with pulmonary TB who met the inclusion criteria. Data on M. tuberculosis bacterial load and TST reactivity were obtained from the Tuberculosis Information System (SITB) at UPTD Puskesmas I, Denpasar Utara District Health Office. Data were analyzed using the chi-square test followed by the Monte Carlo exact test. Results: Most respondents were aged 18 to 59 years (74%) and were male (53%). The bacterial load of M. tuberculosis was predominantly categorized as low (30%), while most household contacts showed negative TST reactivity (78%). Statistical analysis demonstrated a significant relationship between M. tuberculosis bacterial load and TST reactivity (p = 0.001; p < 0.05). Conclusion: There was a significant relationship between the M. tuberculosis bacterial load based on TCM results in patients with pulmonary TB and TST reactivity among household contacts.
