Abstract
EXTENDED-SPECTRUM BETA-LACTAMASE (ESBL) PRODUCING ESCHERICHIA COLI AT PROF. dr. I.G.N.G. NGOERAH GENERAL HOSPITAL ABSTRACT Background: Urinary tract infections (UTIs) caused by Extended-Spectrum Beta-Lactamase (ESBL)-producing Escherichia coli are a major healthcare concern due to increasing antimicrobial resistance and the risk of treatment failure. Objective: This study aimed to determine the antibiotic susceptibility patterns of ESBL-producing Escherichia coli isolated from urine specimens of UTI patients at Prof. dr. I.G.N.G. Ngoerah General Hospital. Methods: A descriptive cross-sectional study was conducted on 148 ESBL-producing Escherichia coli isolates. Secondary data were collected from urine culture, bacterial identification, and antibiotic susceptibility testing performed using the VITEK® 2 Compact system at the hospital's Microbiology Laboratory between January and December 2025. Results: Most isolates were recovered from non-intensive care wards (113; 76.40%), while 35 (23.60%) originated from intensive care wards. All isolates were resistant to Ampicillin, Aztreonam, Ceftazidime, Cefixime, Cefoperazone, Ceftriaxone, Cefuroxime, Cefazolin, and Levofloxacin (100%). The highest susceptibility rates were observed for Meropenem and Tigecycline (100%), followed by Ertapenem (98%), Nitrofurantoin (92.6%), Amikacin (91.9%), and Piperacillin/Tazobactam (83.1%). Conclusion: ESBL-producing Escherichia coli isolates were predominantly recovered from non-intensive care wards, exhibited extensive resistance to several commonly used antibiotics, but remained highly susceptible to carbapenems, Tigecycline, Nitrofurantoin, Amikacin, and Piperacillin/Tazobactam. NGOERAH ABSTRAK Latar Belakang: Infeksi saluran kemih (ISK) yang disebabkan oleh Escherichia coli penghasil Extended-Spectrum Beta-Lactamase (ESBL) merupakan masalah kesehatan yang penting karena dapat meningkatkan resistensi terhadap berbagai antibiotik beta-laktam dan berpotensi menyebabkan kegagalan terapi. Tujuan: mengetahui pola kepekaan antibiotik pada isolat Escherichia coli penghasil ESBL dari spesimen urine pasien ISK di RSUP Prof. dr. I.G.N.G. Ngoerah. Metode: Penelitian dengan desain deskriptif dan pendekatan cross-sectional terhadap 148 isolat Escherichia coli penghasil ESBL. Data diperoleh dari hasil kultur urine, identifikasi bakteri, dan uji kepekaan antibiotik menggunakan VITEK® 2 Compact di Laboratorium Mikrobiologi RSUP Prof. dr. I.G.N.G. Ngoerah periode Januari–Desember 2025. Hasil: sebagian besar isolat berasal dari ruangan nonintensif sebanyak 113 isolat (76,40%), sedangkan dari ruangan intensif sebanyak 35 isolat (23,60%). Seluruh isolat menunjukkan resistensi terhadap Ampicillin, Aztreonam, Ceftazidime, Cefixime, Cefoperazone, Ceftriaxone, Cefuroxime, Cefazolin, dan Levofloxacin (100%). Sensitivitas tertinggi ditemukan pada Meropenem dan Tigecycline (100%), diikuti Ertapenem (98%), Nitrofurantoin (92,6%), Amikacin (91,9%), serta Piperacillin/Tazobactam (83,1%). Kesimpulan: isolat Escherichia coli penghasil ESBL pada pasien ISK didominasi oleh kasus dari ruangan nonintensif, memiliki tingkat resistensi yang tinggi terhadap beberapa antibiotik, namun masih menunjukkan sensitivitas yang baik terhadap beberapa agen antimikroba tertentu. Kata kunci: Escherichia coli ESBL, pola kepekaan antibiotik.
