Abstract

ABSTRACT The process of pregnancy, childbirth, puerperium is a physiological process. If the process is not monitored properly it can turn out to be pathological, thus raising Mortality Rate (MMR) and Infant Mortality Rate (IMR). Various efforts have been made to suppress the AKI and AKB even lower, so that the health status of Indonesia can improve from the results of previous surveys. This author aims to determine the development of pregnancy in the mother "W" age 31 years of primigravida from 37 weeks gestation 3 days to 42 days of puerperium. The method used is by interview, documentation study and observation. Results of care that has been given found the condition of the mother and the pregnancy has a little problem. The well-being of the fetus has problems marked by a high fetal heart rate or tacikardi. Labor took place at 38 weeks 2 weeks of gestation by Sectio Caesarea with an indication of fetal distress. Pathologically involves pathological involution, locia exclusion and lactation during the puerperium are normal. Babies born soon cry and muscle tone well with a weight of 3250 grams. 4950 gram weight gain at 42 days. The conclusion of this final project report is the development of pregnancy mother "W" until 42 days during childbirth walk pathologis but not yet fulfill some standard. Suggestions for service institutions are expected to improve the ability to perform good documentation and correct according to actual patient condition. Keywords: pregnancy, childbirth, childbirth, infant age 42 days ABSTRAK Proses kehamilan, persalinan, nifas merupakan proses yang fisiologis. Jika proses tersebut tidak dipantau dengan baik dapat berubah menjadi patologis, sehingga dapat meningkatkan Angka Kematian IBU (AKI) dan Angka Kematia Bayi (AKB). Berbagai upaya telah dilakukan pemerintah untuk menekan AKI dan AKB lebih rendah lagi, sehingga derajat kesehatan Negara Indonesia dapat mengingkat dari hasil survey sebelumnya. Penulis ini bertujuan untuk mengetahui perkembangan kehamilan pada ibu “W” umur 31 tahun primigravida dari usia kehamilan 37 minggu 3 hari sampai 42 hari masa nifas. Metode yang digunakan adalah dengan cara wawancara, studi dokumentasi dan observasi. Hasil asuhan yan telah diberikan ditemukan kondisi ibu dan masa kehamilan mengalami sedikit masalah. Kesejahteraan janin mengalami masalah ditandai dengan denyut jantung janin yang tinggi atau tacikardi. Persalinan berlangsung pada umur kehamilan 38 minggu 2 hari secara Sectio Caesarea dengan indikasi gawat janin. Proses involusi yang berlangsung secara patologis, pengeluaran lokia dan proses laktasi pada masa nifas berlangsung secara normal. Bayi lahir segera menangis dan tonus otot baik dengan berat badan 3250 gram. Penambahan berat badan 4950 gram pada umur 42 hari. Kesimpulan dari laporan tugas akhir ini yaitu perkembangan kehamilan ibu “W” hingga 42 hari masa nifas berjalan secara patologis namun belum memenuhi beberapa standar. Saran bagi institusi pelayanan diharapkan dapat meningkatkan kemampuan dalam melakukan dokumentasi yang baik dan benar sesuai dengan kondisi pasien sebenarnya.
