Abstract

Asuhan Keperawatan Risko Perfusi Serebral Tidak Efektif Pada Pasien Cidera Kepala Berat Di Instalasi Gawat Darurat RSUD Sanjiwani GianyarTahun 2021 ABSTRAK Cidera kepalamerupakan suatu gangguan traumatic dari fungsi otak yang disertai atau tanpa disertai perdarahan interstiil dalam substansi otak tanpa diikuti terputusnya kontinuitas otak. Tujuan penulisan KIAN ini yaitu untuk menganalisis asuhan keperawatan risiko perfusi serebral tidak efektif pada pasien dengan cidera kepala berat di Instalasi Gawat Darurat RSUD Sanjiwani Gianyar Tahun 2021. Pada tahap pengkajian di dapatkan data 2 kasus kelolaan dengan penyebab cedera kepala berat yang sama yaitu pasien mengalami penurunan kesadaran, peningkatan tanda-tanda vital, kesadaran semikoma, nilai GCS pada pasien 1 GCS :E1V1M4 dan pada pasien ke 2 E1V2M4, pasien tampak gelisah. Berdasarkan pengkajian maka diagnosis keperawatan yag diangkat yaitu risiko perfusi serebral tidak efektif dengan pemberian intervensi 1x2 jam. Intervensi yang diberikan sesuai SIKi yaitu manajemen peningkatan tekanan intracranial dengan pemberian head up 30o.pemberian posisi HEAD UP 300 yaitu memposisikan kepala lebih tinggi sekitar 300dari tempat tidur dengan posisi tubuh sejajar dan posisi kaki atau tidak menekuk. Hasil evaluasi menunjukan bahwa pemberian posisi head up 30o pada pasien cidera kepala dapat menurunkan tekanan intracranial pada pasien cidera kepala. Selain itu posisi tersebut juga dapat meningkatkan oksigen ke otak. Pemberian posisi head up 30o perlu diterapkan untuk meminimalkan risiko pasien mengalami peningkatan tekanan intracranial. Ineffective Cerebral Perfusion Risk Nursing Care in Patients with Severe Head Injury in the Emergency Room Gianyar Sanjiwani Hospital in 2021 ABSTRACT Head injury is a traumatic disorder of brain function with or without interstitial bleeding in the brain substance without being followed by a break in the continuity of the brain. The purpose of writing this KIAN is to analyze the risk of ineffective cerebral perfusion nursing care in patients with severe head injuries in the Emergency Installation of Sanjiwani Hospital, Gianyar in 2021. At the assessment stage, were obtained for 2 cases managed with the same cause of severe head injury, namely the patient experienced a decrease consciousness, increased vital signs, semicoma consciousness, GCS value in patient 1 GCS:E1V1M4 and in patient 2 E1V2M4, the patient looked restless. Based on the assessment, the nursing diagnosis that was appointed was the risk of ineffective cerebral perfusion by giving 1x2 hour intervention. The intervention given according to SIKI is the management of increased intracranial pressure by giving a 30o head up. The head up 30o position is positioning the head about 300 higher than the bed with the body in parallel and the legs or not bending. The results of the evaluation showed that giving the head up position 30o in head injury patients can reduce intracranial pressure in head injury patients. In addition, this position can also increase oxygen to the brain.
