Abstract

Moderate head injury is a condition where there is trauma to the brain and head areas that can cause increased intracranial pressure, causing the risk of ineffective cerebral perfusion. The purpose of this study was to determine how Ineffective Cerebral Perfusion Risk Nursing Care in patients with Moderate Head Injury in the ER Sanglah Hospital. The results of the study were found in the assessment of the two patients managed, namely the patient had decreased consciousness. The formulated diagnosis is Risk for Ineffective Cerebral Perfusion. Nursing planning is prepared based on the reference of the Indonesian Nursing Outcome Standard (SLKI), the Indonesian Nursing Intervention Standard (SIKI) and the Innovation Intervention based on the EBP. The expected outcome after giving nursing interventions for 1 x 4 hours is with an improved Cerebral Perfusion label. The interventions provided according to the SIKI guidelines include the main intervention and the supporting intervention. The main interventions provided were Management of Increased Intracranial Pressure and Monitoring of Increased Intracranial Pressure. While the innovation intervention based on EBP is by giving a 30-degree Head Up Position. Nursing implementation is given for 1 x 4 hours according to a predetermined plan. From the intervention for 1 x 4 hours, the evaluation results showed an increase in decreased consciousness. The provision of an innovative 30-degree Head Up Position intervention is effective in increasing awareness of patients experiencing the risk of ineffective cerebral perfusion.
