Abstract

Hemorrhagic stroke is a rupture of the cerebral blood vessels causing bleeding into the brain tissue or the surrounding area. The prevalence of stroke in Indonesia based on Riskesdas 2018 is 10.9%. The main risk factor that can lead to stroke is hypertension. The purpose of writing this scientific paper is to determine the ineffective Cerebral Perfusion Risk Nursing Care for Patients with Hemorrhagic Stroke in the Emergency Room at Sanglah Hospital, Denpasar, in 2021. The risk of ineffective cerebral perfusion is a risky condition decreased blood circulation to the brain. In this scientific work, there are two patients who are used as managed cases. The results of the study showed that both patients experienced a decrease in consciousness as seen from a decrease in GCS score and had a risk factor hypertension. The nursing diagnosis made in both patients was the risk of ineffective cerebral perfusion as evidenced by hypertension. One of the interventions given is the provision of a 30o head up position, namely positioning the patient's head at a 30o elevation in order to increase venous drainage from the brain and increase oxygenation to the brain. The evaluation obtained after being given nursing care for 1 x 4 hours was that both patients experienced improvements in blood pressure and MAP, improved GCS scores, and decreased levels of anxiety. In Patient II there was a decrease in the pain scale. However, the two patients did not experience a change in the level of consciousness, namely somnolence. So it is suggested to the nurse administrators at the IGD Sanglah Hospital Denpasar to implement a 30o head up position intervention in hemorrhagic stroke patients by minimizing movement in the neck so that venous drainage and perfusion to the brain tissue is maximized.
