Abstract

Chronic Kidney Disease is a process of progressive decline in kidney function and in general to some degree requires renal replacement therapy which remains in the form of dialysis and kidney transplantation. The prevalence of CKD based on the results of basic health research in 2018 reached 19.33%. One of the management of CKD is hemodialysis. The purpose of writing this scientific paper is to determine the Nursing Care for Chronic Kidney Disease Stage V Post Hemodialysis in the Hemodialysis Room of Sanjiwani Hospital. Hemodialysis has the potential to produce complications that can arise during the hemodialysis process, known as intradialytic complications, one of which is fatigue. In this scientific paper, there are two patients who are used as managed cases, namely patients with a medical diagnosis of CKD Stage V and with activity intolerance nursing problems. The results showed that both patients experienced fatigue which could be observed subjectively, and objectively with the patient's vital signs. The nursing diagnosis established in both patients was activity intolerance associated with decreased energy characterized by fatigue. One of the interventions given is Progressive Muscle Relaxation, which is to give a tense effect on the patient's muscles then slowly relax again. The evaluation obtained after being given nursing care for 1x5 hours was that the two patients revealed that their fatigue had improved. In the second patient revealed to be more relaxed. Therefore, it is suggested to consider programmed progressive muscle relaxation exercises in every nursing institution. In addition, it is necessary to make regular progressive muscle relaxation procedures and exercise schedules. For example, with a frequency of 3 times per day after each hemodialysis, it can reduce activity intolerance due to fatigue and improve health.
