Abstract

WHO mengatakan bahwa bayi berat lahir rendah sebagaibayi yang lahir dengan berat badan kurang dari 2500 gram tanpa memandang usia gestasi.Masalah pada bayi BBLR yang sering terjadi adalah gangguan termoregulasi, gangguan pada sistem pernafasan. Tujuan dari penelitian ini adalah menggambarkan asuhan keperawatan pada bayi BBLR dengan Termoregulasi tidak Efektif yang meliputi pengkajian, diagnosis, intervensi, implementasi dan evaluasi. Jenis penelitian yang digunakan merupakan jenis penelitian deskriptif dengan rancangan studi kasus. Subjek yang digunakan yaitu dua subjek yang memenuhi kriteria inklusi dan ekslusi.Hasil penelitian menunjukkan adanya perbedaan dengan teori yang telah disampaikan baik dari pengkajian sampai dengan evaluasi Hasil penelitian ini menunjukkan pengkajian dari kedua dokumen didapatkan hasil data objektif pasien berupa data suhu tubuh pasien dibawah rentang normal,kulit pasien tampak kemerahan. Diagnosis keperawatan yang dirumuskanyaitu hipotermi. Perencanaan keperawatan yang didokumentasikan menggunakan NIC NOC. Implementasi yang dilakukan sesuai dengan intervensi yang telah dirumuskan. Evaluasi keperawatan yang digunakan adalah pendokumentasian SOAP. Berdasarkan observasi adapun saran bagi perawat diharapkan dapat meningkatkan pengetahuan dan keterampilan terhadap acuan terkini sehingga mampu melakukan asuhan keperawatan secara komperehensif dan optimal,diharapkan juga dapat digunakan sebagai bahan untuk menambah wawasan ilmu pengetahuan di bidang keperawatan baik bagi institusi Politeknik Kemenkes Denpasar The WHO says that low birth weight babies are babies born with a body weight of less than 2500 grams regardless of gestational age.Problems with LBW infants that often occur are thermoregulatory disorders, disorders of the respiratory system.The purpose of this study is to describe nursing care in LBW infants with ineffective thermoregulation which includes assessment, diagnosis, intervention, implementation and evaluation.This type of research is a type of descriptive research with a case study design.The subjects used were two subjects who met the inclusion and exclusion criteria.The results showed differences with the theories that have been conveyed both from the assessment to the evaluation of nursing.The results of this study indicate that the first and second document assessment results obtained from the patient objective data in the form of patient body temperature data below the normal range, the patient's skin appears reddish. Nursing diagnoses formulated by nurses in the first document and the second is hypothermia. Nursing planning documented using NIC NOC. The implementation included general implementation and there were only three interventions than had been planned. The nursing evaluation used is SOAP documentation.Based on observations, as for suggestions for nurses, it is expected to be able to increase knowledge and skills against the latest references so that they can carry out comprehensive and optimal nursing care, expected to be used as material to add insight into science in the field of nursing both for the Polytechnic Ministry of Health of Denpasar
