Abstract

Asthma is a reversible and chronic obstuctive airway disease, with characteristics of wheezing, shortness of breath, coughing. The purpose of this study was to determine the description of nursing care in asthmatic children with impaired gas exchange. The research method used is descriptive research and data collection techniques with documentation observation guidelines, then compare the results of observations of the two patient documents with the researchers' reference theory. Case studies were carried out on April 2020. The samples used were 2 people who met the inclusion criteria. The results of the study showed that the assessment of patient documents 1 and 2 obtained objective data, namely the general condition, awareness was compos mentis. Nursing diagnoses formulated by nurses in the room in both documents are ineffective airway cleaning. Interventions carried out in documents 1 and 2 there are differences between nurses in the room with theoretical references. Implementation in both cases is in accordance with the nursing plan. Evaluation of the two patient documents studied there were differences with theoretical references, seen in SOAP, writing of Objective, Assessment, and Planing data, indicating that nursing care documentation carried out was incomplete. The results of this study are expected for the hospital, especially in the Durian Room to apply the appropriate nursing process and document in full according to the guidelines so that the implementation of nursing is carried out optimally. Keywords: Asthma; Gas Exchange Disorders; Nursing Care Asma merupakan penyakit obstuktif jalan nafas yang reversible dan kronis, dengan karakteristik adanya suara mengi, sesak nafas, batuk. Tujuan penelitian ini untuk mengetahui gambaran asuhan keperawatan pada anak asma dengan gangguan pertukaran gas. Metode penelitian yang digunakan adalah penelitian deskriptif dan teknik pengumpulan data dengan pedoman observasi dokumentasi, kemudian membandingkan hasil pengamatan kedua dokumen pasien dengan teori acuan peneliti. Studi kasus yang dilaksanakan pada bulan April 2020. Sampel yang digunakan adalah 2 orang yang memenuhi kriteria inklusi. Hasil penelitian menunjukkan pengkajian dokumen pasien 1 dan 2 didapatkan data objektif yaitu keadaan umum sedang, kesadaran compos mentis. Diagnosa keperawatan yang dirumuskan oleh perawat diruangan pada kedua dokumen yaitu bersihan jalan nafas tidak efektif. Intervensi yang dilakukan pada dokumen 1 dan 2 terdapat perbedaan antara perawat diruangan dengan acuan teori. Implementasi pada kedua kasus sesuai dengan rencana keperawatan. Evaluasi dari kedua dokumen pasien yang diteliti terdapat perbedaan dengan acuan teori, terlihat pada SOAP, penulisan data Objektif, Assesment, dan Planing, menunjukkan bahwa dokumentasi asuhan keperawatan yang dilakukan tidak lengkap. Hasil penelitian ini diiharapkan bagi pihak rumah sakit khususnya di Ruang Durian agar menerapkan proses keperawatan yang tepat dan mendokumentasikan secara lengkap sesuai pedoman sehingga pelaksanaan keperawatan terlaksana dengan optimal.
