Abstract

ABSTRAK Diabetes melitus merupakan penyakit kronis dan progresif yang ditandai dengan peningkatan kadar darah yang dapat menyebabkan gangguan integritas jaringan jika tidak segera diobati. Penelitian ini bertujuan untuk mengetahui gambaran asuhan keperawatan pada pasien diabetes melitus tipe II +diabetic foot dengan Gangguan integritas jaringan meliputi data hasil pengkajian sampai dengan hasil evaluasi keperawatan di Ruang Cendrawasih RSUD Wangaya. Penelitian ini menggunakan jenis penelitian deskriptif dengan Teknik pengumpulan data studi dokumentasi melalui dua dokumen pasien. Hasil penelitian yang didapatkan dari halaman repository Poltekkes Denpasar pada tanggal 27-30 april 2020 yaitu merawat kasus Diabetes Melitus Tipe II+Diabetic Foot dengan Gangguan Integritas jaringan di Ruang Cendrawasih RSUD Wangaya yaitu pengkajian dari dokumen pasien 1 data mayor minor yang tidak muncul yaitu kemerahan dan hematoma. Sementara data mayor minor yang tidak muncul pada pasien 2 yaitu perdarahan, kemerahan, hematoma. Diagnosa keperawatan yang ditegakan pada kedua dokumen yaitu Gangguan Integritas Jaringan berhubungan dengan gangguan sirkulasi. Perencanaan keperawatan yang didokumentasikan mengacu pada NIC NOC. Implementasi yang dilakukan termasuk implementasi secara umum. Evaluasi keperawatan yang digunakan adalah metode pendokumentasian SOAP. Kata Kunci: Gangguan Integritas Jaringan, asuhan keperawatan, Diabetes melitus tipe II+diabetic foot. ABSTRACT Diabetes mellitus is a chronic and progressive disease that is characterized by an increase in blood levels that can cause impaired tissue integrity inf not treated immediately. This research discusses nursing care in patient with type II diabetes mellitus diabetic foot with impaired tissue integrity loading data from assessment results to the evaluation of nursing in Cendrawasih of Wangaya hospital. The research uses descriptive research with documentation study data collection techniques through two patient documents. The result of the research obtained from the Denpasar Poltekkes repository page on 27 to 30 april 2020 are treating cases of diabetic melitus type II + Diabetic Foot with impaired tissue integrity in cendrawasih of wangaya hospital. The assessment of the first patient document that the patient minor mayor data wich do not appear ie redness and hematoma. While the minor mayor data that did not appear in patient two were bleeding, redness and hematoma. Nursing diagnoses are established in both documents, namely impaired tissue integrity associated with circulatory disorders. Documented nursing planning refers to the NIC NOC. The implementation is carried out in accordance with nursing order. The nursing evaluation used is SOAP decomentation.
